Abstract: Objective To study the characteristics of the "matched blood transfusion" in the patients with stem cell transplants of ABO incompatibility.Methods Twenty five casesof ABO blood group incompatibility were detected for their blood group chimericcharacteristics,including the surface antigen of red blood cells (RBCs) and serum antibody alteration. Scheme of transfusion of chimericRBCs andthe effect and adverse reactions following transfusion were evaluated.Results Twenty five patients with stem cell transplants and ABO incompatibility received matched blood transfusion based on the surface antigen of RBCs and serum antibody alteration in chimera,and the transfusion efficacyof 76.38% was obtained. A significantly increased HB levelinpre- and post- transfusions was noted although 3 times of adverse reactions such as non-hemolytic febrile occurred. Conclusion The matched red blood cell transfusion in the patients with chimeric stem cell transplants and ABO incompatibility may help elevate the HB level and ensure the transfusion safety.
