输血前预防性用药对输血不良反应的影响

The Effects of Pre-transfusion Treatment on Adverse Reactions in Blood Transfusion

摘要: 目的 分析输血不良反应的临床特点,探讨输血前用药对预防输血不良反应的作用。方法 分析本院2014年4月~2015年11月报告的输血不良反应信息。根据输血前用药情况,将预防性使用地塞米松的患者设为预防组1,预防性使用异丙嗪的患者设为预防组2,输血前不使用药物的患者设为对照组,比较3组输血不良反应发生情况,并对结果进行统计分析。结果 17 466人次输血治疗的患者中,发生不良反应36例,输血不良反应发生率0.21%,其中过敏反应最多,占不良反应的80.56%;各类血液成分中,单采血小板发生输血不良反应比例最高,发生率为0.58%。预防组1、2和对照组输血不良反应发生率的差异无统计学意义(P>0.05),3组过敏反应及发热反应发生率的差异均无统计学意义(P>0.05)。结论 输血前预防性用药对输血不良反应的发生无明显作用。

Abstract: Objective To analyze the clinical characteristics of adverse transfusion reactions,and investigate the effect of preventive medication on transfusion adverse reactions. Methods Adverse reactions of transfusion were analyzed in the hospital from April 2014 to November 2015. The patients were divided into group 1( dexamethasone before blood transfusions),group 2 (preventive use of promethazine) and control group (patients without any medications). The adverse reactions of three groups were compared,and the results were statistically analyzed. Results There were 36 cases of adverse transfusion reactions among 17 466 blood transfusion cases,with an incidence of 0.21%. Allergic reactions mostly occurred,accounting for 80.56%. Additionally,the incidence of adverse reactions in apheresis platelets transfusion was the highest (0.58%). No significant difference was noted in the incidence of adverse transfusion reactions between group1,group 2,and control group(P> 0.05). Conclusion Preventive medication before blood transfusion has no significant effect on the occurrence of adverse reactions.
