老年尘肺患者肾功能损伤的调查研究

Investigation of Renal Function Injury in Elderly Patients with Pneumoconiosis

摘要: 目的分析老年尘肺患者肾功能情况,评价肾功能损伤的检测指标。方法选取符合入组标准的尘肺患者45例,32例同年龄段正常健康人作为对照组,测定肌酐(Cr)、尿素氮(Bun)、尿酸(Ua)和胱抑素C(Cys-c),根据Cr检测标准,分为肌酐正常组和异常组;比较尘肺组与对照组的Cr、Bun、Ua和Cys-c,并计算尘肺组Cr、Bun、Ua和Cys-c的异常率,比较对照组、肌酐正常组的Bun、Ua和Cys-c。结果与对照组比较,尘肺组Cr、Bun、Ua和Cys-c增高,差异有统计学意义(P<0.05);在尘肺组,异常率从高到低分别为Cys-c、Ua、Bun;与对照组比较,肌酐正常组cys-c增高,差异有统计学意义(P<0.05),Cr 、Bun、Ua的差异无统计学意义(P>0.05)。结论老年尘肺患者易并发肾功能损伤,Cys-c是肾功能早期损伤的敏感指标,有利于早期发现老年尘肺患者的肾功能损伤,为老年尘肺患者肾功能早期损伤的防治及预后提供新的理论基础。

Abstract: ObjectiveTo analyze the renal function of elderly patients with pneumoconiosis, and to evaluate the detection index of renal function injury. Method45 patients definitely diagnosed as pneumoconiosis,and with 32 healthy volunteers as control were collected . All the subjects were detected for creatinine(CR),blood urea nitrogen(BUN),uric acid(UA)and cystatin C (Cys-C). According to the diagnostic criteria of CR, the pneumoconiosis patients were divided into 2 groups: normal and abnormal CR groups. ResultsCompared with the control,the patients with pneumoconiosis had a significant difference in CR,bUN,UA and Cys-C(P<0.05) and the abnormal rates from high to low were Cys-C,UA and bUN. Compared with the control group,Cys-C in the persons with normal creatinine increased(P<0.05);the patients with normal CR had difference high Cys-C level (P< 0.05),but had no difference in CR,bUN,and UA(P >0.05). ConclusionThe elderly patients with pneumoconiosis are predisposed to renal function injury and Cys-C was a sensitive indicator of early renal function damage for the diagnosis of renal disfunction.
