OSTA在老年慢性阻塞性肺疾病患者骨健康评估中的应用*

Application of OSTA to Bone Mineral Density Assessment in Elderly Patients with Chronic Obstructive Pulmonary Disease

摘要: 目的 探讨亚洲人骨质疏松自我筛查工具(Osteoporosis Self-assessment Tool for Asians,OSTA)在老年慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者骨健康评估中的应用前景。方法 依据OSTA风险分层,将120例呼吸系统疾病患者分成两组,其中60例COPD,进行相关特性的分析。结果 老年COPD患者骨质疏松中高风险比例高,达70%,略高于对照组,两组间差异无统计学意义。COPD组中不同风险组间在年龄、体重、身高差、Ⅰ型胶原氨基端前肽(PINP)、深吸气量(IC)及股骨颈部、股骨整体BMD的差异,存在统计学意义。OSTA与骨密度、体重、IC呈正相关,与年龄、身高差、PINP及I型胶原羧基端肽(CTX)浓度、白介素-8(IL-8)、白介素-13(IL-13)、转化生长因子-β1(TGF-β1)水平呈负相关。结论 老年COPD患者骨质疏松中、高风险比例高,OSTA对于该类患者是一种有效、经济、简便的初筛工具。

Abstract: Objective To evaluate the potential use of OSTA in bone density assessment in the elderly patients with chronic obstructive pulmonary disease (COPD). Methods Based on risk stratification of OSTA,a correlation analysis of 120 patients with respiratory diseases were performed,among them 60 cases were COPD. Results No significant difference of osteoporosis at high-risk was seen when compared with the control. The OSTA was positively associated with bone density,weights and IC but negatively correlated to age,height,and concentrations of PINP and CTX,IL-8,IL-13,and TGF-β1. Conclusion The chronic obstructive pulmonary disease is a high risk factor of osteoporosis in senior patients and the OSTA is simple and economic for primary screening of osteoporosis in these patients.
