一氧化氮合酶不同亚型在食管上皮的表达及其与胃食管反流病的相关性

Expression of Nitric Oxide Synthase Different Isoforms in Esophageal Epithelium and its Correlation with the Severity of Gastroesophageal Reflux Disease

摘要: 目的 探讨一氧化氮合酶(NOS)不同亚型在食管上皮的表达及其与胃食管反流病(GERD)病情严重程度的相关性。方法 收集2015年1月~2017年1月在本院接受住院治疗的GERD患者80例作为观察组,同期在本院进行健康体检(包括胃镜)的正常人50例作为对照组。对比两组研究对象食管上皮组织中nNOS、iNOS的表达量,血清胃肠道功能指标、氧化应激指标的含量,食管上皮中增殖基因的表达量,进一步采用Pearson检验评估GERD病变食管上皮中nNOS、iNOS表达量与病情严重程度的相关关系。结果 观察组患者病变食管上皮中nNOS、iNOS mRNA的表达量均明显高于正常对照组(P<0.05);观察组患者血清中胃肠道功能指标胃动素(MTL)、胃泌素(GAS)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)的含量均明显低于正常对照组,血清中氧化应激指标超氧化物歧化酶(SOD)的含量低于正常对照组,脂质过氧化物酶(LPO)、丙二醛(MDA)的含量高于正常对照组(P<0.05);观察组患者病变食管上皮中增殖基因Ki-67、WISP2 mRNA的表达量高于正常对照组,Caspase-3 m RNA的表达量低于正常对照组(P<0.05)。经Pearson检验发现,GERD患者病变食管上皮中nNOS、iNOS表达量与其病情严重程度直接相关。结论 GERD患者病变食管上皮中存在nNOS、iNOS异常高表达,且其具体表达程度与病情严重程度直接相关,是导致疾病发生发展的重要原因之一。

Abstract: Objective To study the expression of nitric oxide synthase(NOS)different isoforms in esophageal epithelium and its correlation with the severity of gastroesophageal reflux disease(GERD). Methods 80 cases of GERD patients in our hospital during January 2015 to January 2017 were chosen as observation group,50 cases healthy people underwent physical examination at the same period were chosen as normal control group. The expression of nNOS and iNOS in esophageal epithelium in two groups were measured,serum contents of gastrointestinal function,oxidative stress were measured,expression of proliferation genes in esophageal epithelium were detected.Relationship between nNOS and iNOS nRNA in GERD patients and those disease related indexes were evacuated by Pearson test. Results the observation group of patients with lesions of esophageal epithelial expression of nNOS,iNOS mRNA in esophageal epithelium in observation group were higher than those in normal control group(P<0.05); serum contents of gastrointestinal function indexes such as MTL,GAS,PG I and PG II were significantly lower than those in normal control group,oxidation stress indexes such as SOD was lower than that in normal control group,LPO,MDA were higher than those in normal control group(P<0.05); expression of proliferation genes such as Ki-67,Caspase-3,WISP2 mRNA in esophageal epithelial were higher than those in normal control group. The Pearson test showed that the expression of nNOS and iNOS in esophageal epithelium in GERD patientswere directly related to the severity of the disease. Conclusion There are abnormally high expression of nNOS and iNOS in esophageal epithelium in GERD patients,and the degree of specific expression is directly related to the severity of the disease. It is one of the important causes of the occurrence and development of GERD.
