Abstract: The Rh blood group system is one of the most clinically significant blood group systems. Incompatibility in this system can lead to alloimmunization, resulting in severe adverse reactions such as hemolytic transfusion reactions in patients and hemolytic disease of the fetus and newborn (HDFN), which threaten patient lives. Recent cutting-edge research on RHD and RHCE variants has yielded significant progress, providing not only an impetus for innovating transfusion protocols but also novel strategies for the management of blood resources. This article reviews the latest advances in the distribution of Rh antigens, their molecular basis, clinical applications, and associated technological innovations, with the aim of optimizing transfusion strategies to enhance safety and efficacy.
