血清可溶性髓系细胞触发受体-1在骨折术后感染中的诊断价值

Diagnostic Value of Soluble Triggering Receptor Expressed on Myeloid Cells-1 on Postoperative Infections Following Fractures

摘要: 目的 探讨血清可溶性髓系细胞触发受体-1(sTREM-1)在骨折术后感染中的诊断价值。方法 将本院骨科收治的187例骨折患者按照术后感染情况分为感染组34例和非感染组153例,分别于术前1 d、术后即刻和术后24 h采集患者肘静脉血5 mL,采用魏氏法测定红细胞沉降率(ESR),免疫透射比浊法测定血清C-反应蛋白(CRP)值,免疫化学发光法检测血清降钙素原(PCT)值,酶联免疫吸附试验检测血清sTREM-1值,利用受试者工作曲线(ROC)比较ESR、CRP、PCT、sTREM-1对骨折术后感染的诊断效能。结果 与术前1 d比较,感染组和非感染组术后ESR、CRP、PCT、sTREM-1持续升高,不同时间点比较差异具有统计学意义(P﹤0.05);感染组术后即刻、术后24 h ESR、CRP、PCT、sTREM-1明显高于同时段非感染组,组间差异亦有统计学意义(P﹤0.05)。术前1 d,ESR、CRP、PCT、sTREM-1诊断术后感染的AUC均低于0.7,准确性较低;术后即刻,PCT诊断术后感染的AUC(0.951)高于ESR(0.830)、CRP(0.738)和sTREM-1(0.840);术后24 h,sTREM-1诊断术后感染的AUC(0.920)高于ESR(0.731)、CRP(0.822)和PCT(0.780)。结论 血清PCT可以作为骨折术后感染早期诊断的指标,sTREM-1可以作为判断术后感染病情和病程的重要指标。

Abstract: Objective To explore the diagnostic value of soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) on postoperative infections following fractures. Methods A total of one hundred and eighty-seven patients with fractures treated in our hospital were divided into infection group (thirty-four cases) and non-infection group (one hundred and fifty-three cases) according to the infection standard,5 mL of veinal blood was collected 1 day before operation,immediate postoperative and 24 h after operation,respectively. The value of ESR was detected by Westergren method,the levels of CRP were measured by immunoturbidimetry assay,the level of PCT was detected by chemiluminescence immunoassay,and the levels of sTREM-1 were measured by enzyme linked immunosorbent assay. The diagnostic value of ESR,CRP,PCT,sTREM-1 for postoperative infections following fractures was performed by receiver operating curve (ROC). Results Compared to the 1 day before operation,the levels of ESR,CRP,PCT,sTREM-1 in the infection group and non-infection group constantly increasing,there was significant difference between different time points (P﹤0.05). The levels of ESR,CRP,PCT,sTREM-1 in the infection group were markly higher than that in the non-infection group,the difference between groups was significantly (P﹤0.05). On the 1 day before operation,the accuracy of ESR,CRP,PCT,sTREM-1 for postoperative infections all were lower (all AUC﹤0.7). At the moment of immediate postoperative,the AUC of PCT (0.951) higher than ESR (0.830),CRP (0.738) and sTREM-1 (0.840). On the 24 h after operation,the AUC of sTREM-1 (0.920) higher than ESR (0.731),CRP (0.822) and PCT (0.780). Conclusion Serum PCT can be used as an index for the early diagnosis of postoperative infections following fractures,while sTREM-1 can be used as an index for disease severity and course of infections.
