我国血小板输注阈值研究的meta分析

A Meta Analysis of Platelet Transfusion Thresholds in China

摘要: 目的 系统评价我国血小板输注阈值的研究结果。方法 检索中国知网、中国生物医学文献数据库、维普中文科技期刊数据库、万方医学网建库至2016年11 月,国内公开发表的纳入不同的血小板输注阈值下的非随机同期对照试验。血小板输注阈值10×109/L为A组、20×109/L为B组,采用RevMan5. 3软件进行meta分析。结果 研究纳入9个非随机同期对照试验,1 520例病例。血小板输注有效率的RR=1.08(95%CI 1.00~1.18),P=0.06。所有出血发生率的RR=1.18(95%CI 0.95~1.47),P=0.14。血小板输注剂量的SMD=-0.79(95%CI -1.59~0.02),P=0.06。血小板输注次数的SMD=-1.21(95%CI -2.12~-0.31),P=0.009。总不良反应发生率的RR=0.55(95%CI 0.38~0.79),P=0.001。结论 对病情稳定、且无明显危险因素的血液病患者,采用 10 × 109/ L 作为血小板输注阈值是安全的。

Abstract: Objective To analyze the research of platelet transfusion cut-off value by meta-analysis in China. Methods Non-randomized concurrent control trials of different platelet transfusion thresholds published in China were searched and retrieved from Cnki.net,SinoMed,VIP and WANFANG DATA by November of 2016. Platelet transfusion threshold of 10×109/L and 20×109/L were designated to A group and B group,respectively. RevMan5. 3 software was applied to the meta-analysis for platelet transfusion effectiveness,bleeding incidence,times and doses of platelet transfusion and adverse reactions in patients of the two groups.Results Nine studies were selected for meta-analysis,covering a total of 1520 patients.Platelet transfusion efficiency was [RR=1.08,95%CI (1.00,1.18),P= 0.06]; total bleeding incident rate was [RR=1.18,95%CI (0.95,1.47),P=0.14]; platelet transfusion dose was [SMD=-0.79,95%CI(-1.59,0.02),P=0.06]; frequency of platelet transfusion was [SMD=-1.21,95%CI (-2.12,-0.31),P=0.009]; and the total adverse reaction rate was [RR=0.55,95%CI(0.38,0.79),P=0.001]. Conclusion It is relatively safe for the patients with hematologic diseases,in stable conditions and with no obvious risk factors,to take 10×109/L as a prophylactic platelet transfusion threshold.
