支气管哮喘患儿白介素、TNF-α与肺功能及免疫功能的关系研究

Study on Relationship between Interleukin,TNF-αand Lung Function and Immune Function in Children with Bronchial Asthma

摘要: 目的 研究支气管哮喘患儿白介素和TNF-α与肺功能、免疫功能的关系。方法 将178例支气管哮喘患儿分为发作组(急性发作期,107例)及缓解组(缓解期,71例),另取同期体检健康者83例作为对照组。均检测白介素[白介素-2(IL-2)、白介素-4(IL-4)、白介素-6(IL-6)]、肿瘤坏死因子-α(TNF-α)、肺功能[用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、FEV1/FVC、呼气峰值流速(PEF)]及免疫功能[T淋巴细胞亚群(CD4+、CD8+、CD4+/CD8+)],并采用Pearson相关性分析法分析血清炎性因子水平与肺功能、免疫功能的关系。结果 血清IL-2、IL-4、IL-6及TNF-α水平以发作组最高,缓解组次之,对照组最低,差异显著(P<0.05);FVC、FEV1、FEV1/FVC、PEF、CD4+、CD4+/CD8+水平以发作组最低,缓解组次之,对照组最高,差异显著(P<0.05);CD8+水平在各组间无明显差异(P>0.05);经Pearson相关性分析,发作组和缓解组血清IL-2、IL-4、IL-6、TNF-α水平与FVC、FEV1、FEV1/FVC、PEF、CD4+、CD4+/CD8+均呈负相关(P<0.05),与CD8+无相关性(P>0.05)。结论 支气管哮喘患儿白介素和TNF-α显著升高,肺功能及免疫功能均明显降低,且白介素和TNF-α与肺功能及免疫功能呈负相关性,临床中可将以上指标作为评估支气管哮喘患儿治疗效果的重要依据。

Abstract: Objective To study the relationship between interleukin,TNF-αand lung function and immune function in children with bronchial asthma.Methods 178 children with bronchial asthma were divided into acute attack group(n=107) and remission group(n=71),and 83 healthy persons at the same period were enrolled in control group.The serum levels of interleukin [interleukin-2(IL-2),interleukin-4(IL-4)and interleukin-6(IL-6)],tumor necrosis factor-α(TNF-α),lung function related indicators [forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),FEV1/FVC and peak expiratory flow(PEF)] and immune function related indicators [T lymphocyte subsets(CD4+,CD8+andCD4+/CD8+)] were measured.Pearson correlation analysis was used to analyze the relationship between serum inflammatory factors and pulmonary function and immune function.Results The serum levels of IL-2,IL-4,IL-6 and TNF-αwere significantly increased in attack group(P<0.05).The levels of FVC,FEV1,FEV1/FVC,PEF,CD4+and CD4+/CD8+ were significantly decreased in attack group(P<0.05).There was no significant difference in the levels of CD8+ among groups(P>0.05).Pearson correlation analysis showed that serum levels of IL-2,IL-4,IL-6 and TNF-αwere negatively correlated with FVC,FEV1,FEV1/FVC,PEF,CD4+,CD4+/CD8+ in attack group and remission group(P<0.05),but no with CD8+(P>0.05).Conclusion The levels of interleukin and TNF-αin children with bronchial asthma are significantly increased,while lung function and immune function are significantly reduced.Moreover,interleukin and TNF-αhave a negative correlation with lung function and immune function.The above indicators can be used to evaluate the therapeutic effect of bronchial asthma in children.
