Regulation of Ubiquitin Mediated Proteolytic Pathway: Predictive Analysis of CircRNA Changes in Stored Red Blood Cells

Objective To investigate potential mechanism of circRNA regulating ubiquitin proteolytic pathway in stored red blood cells (RBCs) and its possible role in RBCs storage lesion. Methods The suspended RBCs (SRBCs) were collected from three healthy donors. After white blood cell (WBC) filtration,SRBCs were divided into fresh group (0-Day)group,stored RBCs for 20 days (20-Day) group and stored RBCs for 40 days(40-Day) group at 4℃. After high-throughput sequencing,Kyoto Encyclopedia of Genes and Genomes(KEGG)and Gene ontology (GO) analysis were performed. Selected circRNAs of parental gene in ubiquitin mediated proteolysis were verified by PCR. These circRNAs were annotated and targeted prediction by circBase database and mirTarBase. Then,analysis of miRNA-mRNA interactions was also performed. CircRNA interactome software was used to predict RNA-binding proteins(RBPs)on circRNAs. Results Compared to fresh group,the expression of hsa_circ_0036044,hsa_circ_0024173 and hsa-miR-4530 increased in 20-Day group,with a statistically significant(P<0.05); while the expression of hsa_circ_0035761 and TRAF2 decreased, with a statistically significant(P<0.05). Compared to fresh group,hsa_circ_0002502 decreased in 40-Day group,with a statistically significant(P<0.05). Compared to the 20-Day group,hsa_circ_0024173 and hsa-miR-4530 decreased in 40-Day group,with a statistically significant(P<0.05); while the expression of hsa_circ_0028196,hsa_circ_0035761 and TRAF2 increased,with a statistically significant (P<0.05). Bioinformatics analysis showed that hsa_circ_0035761 might be involved in physiological processes mediate anti-apoptosis. Conclusion We found that multiple circRNA expressions were difference changes in ubiquitin-mediated proteolytic pathway,and also constructed the hsa_circ_0035761—hsa-miR-4530—TRAF2 axis. It revealed that hsa_circ_0035761 might have an anti-apoptotic effect on physiological processes. These circRNAs had ability to interact with many RNA-binding proteins,and this might be the main mechanism for high level expression of these circRNAs.

Study on Decreased Expression of CircRNAs in Stored Erythrocytes and Their Target Genes Prediction

Objective To investigate the relationship between significant decreased expression of circRNAs and its predicted target genes in erythrocyte storage lesion. Methods Whole blood collected from healthy donors is separated to suspended red blood cells after leukocyte filtration. The samples were divided into fresh groups(0 day)and storage groups(4℃ for 20 days). The two groups of specimens were tested by high-throughput sequencing,and 10 down-regulated circRNAs were selected for PCR verification. CircBase and mirTarBase were used to database annotation and target gene prediction,and the association analysis of mRNAs which could be effected by miRNAs were also be done. Storage lesion-related circRNAs could be screened out through methods above. Results Compared to the fresh groups,the expression of hsa_circ_0005413,hsa_circ_0008937,hsa_circ_0040340,hsa_circ_0003126,hsa_circ_0007995,hsa_circ_0023937,
novel_circ_0000402,hsa_circ_0028281 significantly decreased in storage groups(P<0.05),while the expression of hsa-miR-7160-5p,hsa-miR-122-5p and hsa-miR-4534 significantly increased(P<0.05)and the expression of CASP8,CDK4,and TMOD3 decreased significantly(P<0.05). Bioinformatics analysis shows that these circRNA target genes are involved in apoptosis and anti-apoptosis,erythrocyte membrane stability,oxidative damage,iron metabolism and other physiological processes. Conclusion We constructed several differentially expressed circRNA-miRNA-mRNA triple network,and found that hsa_circ_0005413-hsa-miR-7160-5p-CASP8,hsa_circ_0040340-hsa-miR-122-5p-CDK4,hsa_circ_0007995-hsa-miR-4534-TMOD3 axis might participate in the biological process of erythrocyte storage lesion.

Evaluation of Leukocyte-reduced Suspended Red Blood Cells Stored at 4 ℃ for Different Times

Objective We evaluated the quality of leukocyte-reduced suspended red blood cells (SRBCs) with storage time by detecting hemolysis rate, bacterial growth, biochemical indicators and indexes of energy metabolism, membrane damage and oxidative stress at 4 ℃. Methods On the day of collection, 50 mL of whole blood was collected from 10 volunteers as test specimens. Leukoreduction filters were used for the removal of leukocytes and the plasma was removed as much as possible on the day of collection. We added erythrocyte preservation solution according to a ratio of preservative solution per 200 mL of whole blood and stored at 4℃ until the end of study. The hemolysis rate, ATP/2, 3-DPG, Na+/K+/GLU/LDH/pH, PS/CD47 and MDA/SOD were tested on day 0, 7, 14, 21, 28, 35 and 42 of storage. Bacterial growth was measured on day 0, 14, 28 and 42 of storage. Results The hemolysis rate progressively increased with storage, which increased from 0.03% to 0.4% at from day 0 to 42. There were statistically significant differences compared to day 0 (P<0.001), but none of them exceeded the national standard requirements. There was no bacterial growth. Na+, GLU and pH decreased with increasing K+ and LDH levels in supernatant. ATP began to decrease on Day 14, dropping to 36.4% on Day 21 and to 13.2% at the end. There were significant differences compared to day 0 (P<0.001). 2, 3-DPG began to decline on day 7, decreased to 57.5% on day 28 and 44.0% on day 42. At the end of storage, external exposure of membrane phosphatidylserine (PS) increased to 0.83%, and the mean fluorescence intensity of CD47 decreased to 60.8%. There was no change in MDA value during storage. Activities of SOD decreased on day 28, with statistical difference compared to Day 0 (P<0.001). Conclusion By evaluating the quality of leukocyte-reduced SRBCs stored at 4 ℃, we suggested that the optimal storage period of leukocyte-reduced SRBCs was within two weeks, because indexes of biochemical, energy metabolic and oxygen-carrying capacity were basically maintained in two weeks.

Effect of Frozen Storage Platelet-rich Plasma on Inflammatory Response of IL-1β-I Induced Chondrocyte

Objective The aim of the present study is to investigate whether frozen could influence platelet-rich plasma (PRP) releases growth factors, and whether frozen PRP could protect chondrocyte from IL-1β-induced injury. Methods PRP concentrates were obtained from healthy subjects by apheresis and were divided into 4 groups, then were frozen for as long as 3 months. The storage condition was -20 ℃ without preservation agent, -80 ℃ without preservation agent, -20 ℃ in 5% DMSO and -80 ℃ in 5% DMSO. The growth factors (PDGF-AB, VEGF and IGF-1) in PRP releasates were quantified by ELISA. The effect of PRP on the proliferation of IL-1β induced chondrocytes’inflammation was detected by CCK-8. The levels of IL-6 in cells were detected by ELISA. Results No significant difference was observed in PDGF-AB levels among fresh PRP group and frozen PRP groups. VEGF and IGF-1 levels were significantly decreased in the PRP samples stored at frozen conditions. No obvious difference with CCK-8 assay was observed between the PRP stored in -80 ℃ in 5% DMSO group and fresh PRP group. They both can promote IL-1β-induced inflammatory chondrocyte' s proliferation. The other three experimental groups can’t promote IL-1β-induced chondrocyte proliferation. After PRP treatment, IL-6 production of inflammatory chondrocyte was decreased. No statistically significant differences were found in IL-6 production between fresh PRP and frozen PRPs. Conclusions ome growth factor levels of PRP were reduced after frozen storage. But the frozen PRP can protect IL-1β-induced chondrocyte injury by inhibiting inflammatory response.

Role and Mechanism of AMPK Signaling Pathway in the Development of Acute Kidney Injury in Mice

Objective To investigate the role and mechanism of Adenosine phosphate activating protease (AMPK) signaling pathway in the development of acute kidney injury in mice. Method 48 SPF BALB/C male mice were randomly divided into two groups:control group(n=24) and experimental group(n=24). The experimental group were treated with adefovir dipivoxil 40 mg/kg,once a week for 2 times,while the control group were treated with equal amount of isotonic saline. Serum urea nitrogen(BUN)and creatinine(SCr) were measured by automatic biochemical analyzer. The expression of AMPK and Keap-1 in renal tissue were detected by Western blotting,and histopathological scores were also obtained. Results After 2 weeks,4 weeks,and 6 weeks of treatment,serum BUN and SCr values in the experimental group were significantly higher than those in the control group(P<0.05),the histopathological scores of the kidneys in the experimental group were significantly higher than those in the control group (P<0.05),and the relative expression levels of AMPK and Keap-1 protein in the kidneys of the experimental group were significantly higher than those in the control group (P<0.05). Conclusion Acute kidney injury in mice can activate the AMPK signaling pathway and induce the expression of AMPK and Keap-1 increase,which leads to the deterioration of renal tissue and renal function.

A Prospective Multi-center Study on the Relationship between the Age and the Agglutination Intensity of Anti-A/B Antibody in Infants within One Year of Age

Objective A prospective multi-center study was conducted to investigate the correlation between agglutination intensity of Anti-A/B antibody and age in infants <1 year of age. Methods 1 564 infants were divided into 12 groups by age. The ABO reverse typing was respectively detected by tube method at room temperature and at 4℃ and microcolumn gel method. Results Considering the results of reverse typing by tube method at 4 ℃ and microcolumn gel method, it showed that agglutination score ≥ 4 points (agglutination ≥ 1+W) in the infants age ≥6 months and that agglutination score ≥7 points (agglutination ≥2+W) in the infants age ≥ 9 months. Considering the results of reverse grouping by test tube method at room temperature, it showed that the agglutination score ≥4 points (agglutination ≥1+W) in the infants age ≥ 8 months, and that the agglutination score ≥7 points (agglutination ≥2+W) in the infants age ≥ 10 months. In addition, the agglutination scores (A cell and B cell) of group O infants at 1-month old were higher than group B infants (A cells) and group A infants (B cell) respectively in reverse grouping. In regards of group O infants, the agglutination scores of A cells and B cells reached to the nadir at the age of 4 months, then gradually increased. Conclusion We recommended the detection time and agglutination intensity value of ABO reverse typing in Chinese infants within 1 year old.

Serological Characteristics of Blood Group and Efficacy of Suspended Red Blood Cells Transfusion in Patients with Autoimmune Hemolytic Anemia

Objective To analyze the serological characteristics of blood group in patients with autoimmune hemolytic anemia (AIHA) and the therapeutic effect and applications of suspended red blood cells (SRBCs). Methods Fifty-six patients with AIHA hospitalized in our hospital from January 2014 to December 2019 were selected. The serological characteristics of blood group in 56 patients were analyzed retrospectively. The association between positive direct antiglobulin test (DAT), agglutination strength and hemolytic intensity and degree of anemia were analyzed. Patients were divided into three groups: group A (8 cases, with drug), group B (18 cases, with washed RBCs) and group C (30 cases, with SRBCs). The treatment effect and the changes of RBCs, hemoglobin (Hb), reticulocyte (Ret) and total bilirubin (TBIL) were compared before and after treatment. The clinical symptoms and adverse reactions were compared among the three groups. Result Among 56 patients,34 (60.71%) were anti-IgG autoantibodies, 5 (8.93%) anti-C3d autoantibodies and 17 (30.36%) anti- IgG+ C3d mixed autoantibodies. There was significant difference in hemolytic parameters between simple type (IgG, C3d) and complex type (IgG+ C3d) (P<0.05). Fifty-six patients had positive DAT (agglutination strength W + - 4+). Autoantibodies and DAT agglutination strength were positively correlated with hemolysis and anemia (P<0.05). After treatment, Ret% and TBIL were significantly decreased, and RBC and Hb were significantly increased (P<0.05). There was no significant difference in RBC, Hb, Ret and TBIL among the three groups before and after treatment (P>0.05). Based on the total effective rate in group A (87.5%), group B (94.4%), and group C (100%) after treatment, there was no significant difference in the effective rate of complete and partial of clinical symptoms among the three groups (P>0.05). Conclusion The stronger the agglutination intensity of DAT and autoantibody in AIHA patients, the more severity of hemolysis and anemia. AIHA patients treated with SRBCs had definite effect, which could obviously improve lab tests index and improve the patient's anoxia symptom. It has safety, no adverse reactions, and has clinical significance. It is worth popularizing and suitable for clinic.

Correlation between Peripheral Blood Neutrophil Lymphocyte Ratio and the Efficacy of Blood Transfusion in Patients with Traumatic Blood Loss

Objective To analyze the correlation between the ratio of neutrophil lymphocytes in peripheral blood and the effect of blood transfusion in patients with traumatic blood loss. Methods This was a retrospective analysis of 568 traumatic blood loss patients admitted to our hospital from January 2017 to December 2019, which were divided into effective group (n=500) and ineffective group (n=68) according to the effect of blood transfusion. The general information such as age and gender of the two groups of patients, and the commonly used laboratory indicators such as hemoglobin (Hb), Neutrophil lymphocyte ratio (NLR) and so on were compared. Multi-factor Logistic regression was used to analyze the above factors, and the receiver operating characteristic curve (ROC curve) was used to analyze the diagnostic value of related factors to predict the efficacy of transfusion. Results The age, ISS score and NLR level of the patients in the effective group were significantly lower than those in the ineffective group. The levels of SBP, DBP, respiratory rate, GCS score, Hb, PLT and APTT were significantly higher than those in the ineffective group (all P<0.05); There were no significant difference among the patients' gender, trauma type, and other factors (all P>0.05); multivariate logistic analysis showed high GCS values (OR=0.804, 95% CI: 0.707-0.913) and high Hb (OR=0.937, 95% CI: 0.908-0.967), PLT level (OR=0.984, 95% CI: 0.976-0.992) were protective factors affecting the efficacy of transfusion, while high ISS value (OR=1.288, 95% CI: 1.128-1.471), high NLR (OR=4.434, 95% CI: 2.905-6.766) and APTT level (OR=1.087,95% CI: 1.033-1.145) were independent risk factors affecting the efficacy of blood transfusion; ROC curve showed that the diagnostic efficacy of NLR was significantly higher than that of GCS, ISS, Hb, APTT, and PLT (Z=3.627, 3.487, 2.860, 3.903, 3.030; all P<0.05). The optimal cut-off point for NLR diagnosis was 4.41. At this time, the sensitivity was 86.8% and the specificity was 67.0%. Conclusion NLR was a highly valuable predictor of transfusion efficacy in patients with traumatic blood loss.

Factors for Adverse Reactions to Platelet Transfusions in Tumor Patients

Objective To discuss the factors associated with adverse reactions caused by platelet transfusions in patients with tumors. Methods A retrospective analysis of adverse reactions in tumor patients receiving platelet transfusions in a hospital between January and December 2018 was done. Patients were divided into adverse reaction group and non-adverse reaction group. Univariate analyses were conducted to assess the factors in the two groups, such as age, sex, ethnicity, blood type, history of pregnancy, history of blood transfusion, clinical diagnosis, platelet transfusion dosing, platelet storage time, platelet transfusion times and compatible platelet transfusions etc.. Logistic regression analysis was conducted to analyze the independent risk factors affecting adverse reactions during platelet transfusion. Results A total of 1 086 patients receiving platelet transfusions were included in the study. The rate of adverse reactions to transfusion was 6.72% (73/1 086), including 36 patients with non-hemolytic fever and 37 patients with anaphylaxis. There were statistically significant differences between the two groups in transfusion history, platelet storage period, platelet transfusion times and HLA/HPA-matched platelet transfusions (P<0.05). Multivariate regression showed HLA/HPA-unmatched platelet transfusions (random transfusions)(OR=2.113,95%CI:1.282~3.482,P=0.003)was an independent risk factor for adverse reactions to platelet transfusion. Conclusion HLA/HPA-matched platelet transfusions can reduce the incidence of adverse transfusion reactions in tumor patients.

The Effect of Donation Frequency on Serum Calcium, Magnesium and Parathyroid Hormone Levels in Plateletpheresis Donors

Objective To understand change of serum magnesium during plateletpheresis and the effect of donation frequency on serum calcium, magnesium and parathyroid hormone (PTH) in regular plateletpheresis donors. Methods A total of 29 plateletpheresis donors were divided into two groups based on their donation frequency: high frequency (n=16,>12 times per year), low frequency (n=13, ≤12 times per year). Serum levels of calcium, magnesium and PTH in donors at 0 min, 30 min, 60 min, 90 min of plateletpheresis were measured. The differences of the three markers between groups was analyzed by analysis of variance of repeated measurement data. Results The serum calcium(mmol/L)of donors in low frequency group and high frequency group at 0 min, 30 min, 60 min, 90 min of plateletpheresis were 2.41±0.23 vs. 2.40±0.13、2.17±0.22vs. 2.26±0.11、2.20±0.24 vs. 2.27±0.14、2.21±0.22 vs. 2.28±0.18, respectively, and all of which decreased compared to the beginning of plateletpheresis, and the decreased levels was at 30 minutes, 60 minutes, and 90 minutes after starting plateletpheresis. Serum magnesium (mmol/L) were 0.76±0.18 vs. 0.75±0.21、0.69±0.21vs. 0.71 ± 0.21、0.70±0.19 vs. 0.72±0.19、0.71±0.20 vs. 0.73±0.18, respectively. The change trend was similar to that of serum calcium. Serum PTH (pg/mL) were 40.32±17.24 vs. 60.35±58.80、138.87±63.55 vs. 190.43±64.85、129.14±65.70 vs. 162.68±47.19、89.00±63.85 vs. 122.22±57.90, respectively, and all of which increased compared to the beginning of plateletpheresis. The increased levels was at 30 minutes, 60 minutes, and 90 minutes after starting plateletpheresis. Only the PTH levels between the two groups were 138.87±63.55 vs. 190.43±64.85 at 30 min of plateletpheresis (P<0.05). Conclusions erum magnesium decreased, and PTH in donors with lower donation frequency significantly increased during plateletpheresis, which should arouse more attention of blood collection and supply institutions.

Cytogenetic and Molecular Genetic Studies on Chronic Lymphocytic Leukemia with t(18;22)(q21;q11): Three Cases

Objective To analyze clinical characteristics and laboratory features of chronic lymphocytic leukemia (CLL) with t(18;22)(q21;q11). Methods CLL cells with DSP30+IL2 stimulant were cultured for 72 hours. Chromosome preparation and karyotype analysis were performed by routine method and R-banding technique. Furthermore, BCL-2 rearrangement in 3 cases was confirmed by fluorescence in situ hybridization(FISH). Meanwhile,we presented a whole genome sequencing(WGS) of case No.1. Patients were followed-up for chemotherapy efficacy and survival. Results Abnormal karyotype described as t(18;22)(q21;q11) were showed in all 3 patients. FISH analysis revealed BCL2 rearrangement. WGS confirmed a BCL-2 and IGL translocations in case No.1. Conclusion t(18;22) has different molecular structure from t(14;18)and the incidence of abnormality is low. It is more common in CLL. CLL with this abnormality may be refractory and easy to relapse.

Objective To explore the immunophenotype characteristics and clinical significance of circulating tumor plasma cell (CTPC) in patients with multiple myeloma(MM). Methods 75 newly diagnosed MM patients in our hospital were retrospectively analyzed. The immunophenotype and quantitative correlation of peripheral blood (PB) and bone marrow(BM) samples were detected by ten color flow cytometry. The MM patients were divided into CTPC positive group(CTPC+)and CTPC negative group (CTPC-). Results There was a positive correlation between CTPC and the proportion of mono-clone plasma cells in BM(γ=0.536 8,P<0.000 1),but there was no significant difference between CTPC+ group and CTPC- group(P=0.08).Compared with CTPC- group,the expression of CD19 in mono-clone plasma cells in CTPC+ group was significantly decreased(P=0.029),the expression of CD81 was significantly increased(P=0.006),and the expression of Kappa,Lambda,CD56 and CD117 was not significantly different(P>0.05). There was no significant difference between CTPC and BM mono-clone plasma cells in the expression of Kappa,Lambda,CD19,CD56,CD81 and CD117(P>0.05). The serum level of β2-MG and the number of genetic high risk groups in CTPC-group were significantly lower than those in CTPC+ group,but there was no significant difference in CR between the two groups. Conclusion CTPC could reflect the load of intramedullary tumor cell. The low expression of CD19 and high expression of CD81 in BM clonal plasma cells are correlated with BM infiltration into peripheral system.

Correlation Study of CXCL12/CXCR4 with Preeclampsia and Abnormal Liver Function

Objective To compare the serum chemokine CXCL12/CXCR4 levels between patients with preeclampsia and normal pregnant women, and ananlyze its correlation with abnormal liver function of pregnant women with preeclampsia. Methods 65 patients with preeclampsia (PE group) and 51 normal pregnant women (control group) who underwent cesarean section in the affiliated Maternal and Child Health Hospital of Anhui Medical University from May 2018 to May 2019 were enrolled. Patients in PE group were further divided into mild PE group (23 patients) and severe PE group (42 patients). We compared general clinical data,serum CXCL12/CXCR4,ALT and AST levels among three groups, and evaluated the correlation between the serum CXCL12/CXCR4 levels and abnormal liver function in pregnant women with preeclampsia and calculate their sensitivity and specificity. ROC curve was used to evaluate the accuracy of this prediction model. Results The gestational age of patients and birthweight of newborns in severe PE group were significantly lower than those in control group and mild PE group (P<0.001). Pairwise comparison of the maternal serum CXCL12/CXCR4 levels of the control group, mild PE group and severe PE group were statistically different,and gradually decreased (all P<0.017). The serum ALT of severe PE group was significantly higher than that of mild PE group and control group, while the serum AST of severe PE group and mild PE group was significantly higher than control group(P<0.017). ROC curve analysis results showed that the area under the ROC curve (AUC) for the diagnosis of preeclampsia was 0.981 with serum CXCL12 as the best cut-off value of 2477.68pg/mL [sensitivity was 92%;specificity was 94% (95%CI: 0.963-0.999)],and AUC was 0.996 withserum CXCR4 as the best cut-off value of 2125.97pg/mL [sensitivity was 95.4%; specificity was 100% (95%CI: 0.960-1.000)]. Spearman correlation analysis showed that maternal serum CXCL12 level was negatively related with ALT(r=-0.486) and AST(r=-0.386), and CXCR4 level was also negatively related with ALT(r=-0.474)and ASTr=-0.403)(P<0.001). Conclusion Decreased maternal serum CXCL12/CXCR4 levels are related with preeclampsia and may be involved in the pathogenesis of abnormal liver function in pregnant women with preeclampsia.

Comparison of the Clinical Efficacy of Quinolones, β- lactams Combined with Quinolones and Carbapenems in the Empirical Treatment of Severe Community Acquired Pneumonia

Objective To explore the efficacy of quinolones alone, combined with β-lactam ceftriaxone and carbapenems alone in the treatment of patients with severe community acquired pneumonia. Methods A total of 105 severe community-acquired pneumonia cases in the department of intensive care medicine from June 2016 to August 2020 were enrolled in our hospital. They were divided into three groups according to their use of antibiotics. The first group were treated with moxifloxacin (quinolone drug), the second group patients were treated with ceftriaxone combined with moxifloxacin (β-lactam antibiotic combined with quinolone), and the third group patients were treated with imipenem (carbapenem antibiotic) as a single agent. We compared the total effective rate of the three groups of treatment, the use time of antibacterial drugs, the improvement time of clinical symptoms (cough, sputum, fever and pulmonary rale), mechanical ventilation time, the length of stay in ICU, and the changes in the levels of inflammation indicators (WBC, CRP and PCT). Results Among 105 patients, there was no significant difference in total effective rate between the first group and the second group(χ2=8.274,P= 0.116) and between the second group and the third group (χ2=0.311,P=0.856), but statistically significant difference between the first group and the third group (χ2=10.679,P=0.005). In terms of clinical indicators, patients of three groups were all improved. There was statistically significant difference in all clinical indicators between the first group and the second group(P<0.05); Except for the length of stay in the ICU, the difference was statistically significant in other indicators between the first group and the third group(P<0.05), and only the length of stay in ICU was statistically different between the second group the third group(P<0.05). The inflammatory factors of the three groups were reduced after treatment(P<0.05). The reduction of blood WBC and PCT level showed statistically difference between the first group and the second group(P<0.05); The reduction of the blood WBC and CRP in the first group compared with the third group; Only the reduction of blood PCT level were statistically significant in the second group compared with the third group (P<0.05). Conclusion In the treatment of severe community-acquired pneumonia, compared with moxifloxacin alone, moxifloxacin combined with ceftriaxone, and imipenem alone have better effect.

Correlation between Serum Gd-IgA1 Level and Clinical and Pathological Characteristics of Patients with IgA Nephropathys

Objective To observe the relationship between serum Gd-IgA1 level and clinicopathological features as well as hormone response in patients with IgA nephropathy. Methods The perspectively review identified 40 patients with IgAN were confirmed by renal biopsies as experimental group, and these patients have been treated with hormones therapy in our department. And 20 patients with membranous kidney disease and 20 normal healthy controls in physical examination center of our hospital were regarded as control groups. The level of Gd-IgA1 in all serum samples and the general data of patients, renal function, 24 h urinary protein quantity and IgA level were statistically analyzed. The pathological sections of IgA nephropathy were graded by Haas, and the results of grading were statistically analyzed. Results Compared with normal group (3.56±0.30) mg/L and disease control group (3.40±0.41) mg/L, the level of serum Gd-IgA1 in patients with IgA nephropathy(9.44±0.74)mg/L was higher, and was positively correlated with 24 h urinary protein quantity and renal pathological grade, and the difference was statistically significant (P<0.05). After 1 month of hormonal therapy, the level of Gd-IgA1 in IgAN patients was lower than before(P<0.05). Conclusion The level of serum Gd-IgA1 was positively correlated with the severity of IgA nephropathy, and decreased after hormone treatment, suggesting that serum-Gd-IgA1 could serve as a predictor of renal outcomes in IgAN. Thus, Gd-IgA1 could be significant biomarker for patients with IgAN.

Objective To investigate the effects of fecal micro-ecological transplantation combined with sulfasalazine on serum inflammatory factors and T-cell subsets in patients with ulcerative colitis. Methods a study of 90 patients with UC admitted to our hospital from June 2018 to December 2019, the observation group was given fecal microecological transplantation combined with sulfasalazine, and the control group was given conventional treatment to observe serum inflammatory factors, Mayo score, T cell subsets and clinical efficacy before and after treatment in UC patients. Results The effective rate of the observation group was 95.5%. The effective rate of the observation group was significantly better than that of the control group, and the Mayo score was significantly lower than that before treatment (P<0.05). Inflammation of CRP, TNF-α, IL-6, etc. The factors were significantly decreased (P<0.05), and better than the control group (P<0.05). After treatment, the CD4+ increased and the CD4+/CD8+ ratio increased (P<0.05). The observation group was significantly higher than the control group (P<0.05). Conclusions FMT combined with sulfasalazine can improve the immune status of T cell subsets, reduce the inflammatory response, and achieve the effect of treating UC by establishing the normal intestinal flora. It has certain safety.

Correlation Analysis of Antimicrobial Resistance and Resistance Genes of Carbapenem-resistant Klebsiella Pneumonia

Objective To analyze the distribution of resistance genes of carbapenem-resistant Klebsiella pneumoniae (CRKP) in our hospital and the relationship between resistance genes and phenotypes. The results will provide reference for clinical treatment, prevention and control. Method The non-duplicated CRKP28 strains isolated from clinical specimens were collected. The bacteria were identified and tested for drug sensitivity by VITEK-2 compact automatic microorganism identification and drug susceptibility analyzer. The whole genome sequencing technology was used for sequencing. Results 47 drug resistance genes were detected. There were 3 carbapenem resistance genes, mainly blaKPC-2 gene (25 strains,89.29%),blaNDM-5 gene(2 strains,7.14%) and blaOXA-48 gene(1 strain, 3.57%); Extended-spectrum β-lactamases(ESBLs) resistance genes were mainly blaTEM-1,blaSHV-28,blaCTX-M-15 and blaOXA-33, and the detection rates were 89.29%,75. 00%,75.00% and 67.86%, respectively. AAC (6 ') -Ib-cr and AAC (3) -IIa were the main aminoglycoside resistance genes,the detection rates were 71.43% and 67.86%,respectively; oqxA and AAC (6 ') -Ib-cr were the main quinolone resistance genes, and the positive rates were 85.71% and 71.43%,respectively. Among them, 20 CRKP strains carried both oqxA and AAC (6 ') -Ib-cr; FosA6 was the resistant gene to fosfomycin, and the positive rate was 100%. There were three sulfonamides resistance genes, mainly sul2 and sul1,and the detection rates were 21.43% and 17.86%,respectively. The drug resistance of 28 strains of CRKP to carbapenems,cephalosporins, quinolones and β-lactams/β-lactamase inhibitor mixture was 100%; The resistant rate to amikacin was 14.29%,and no resistant strain to tigecycline was found. The resistance phenotype and genotypes of 28 strains of CRKP were completely consistent with the tested β-lactam antibiotics. Conclusion CRKP isolates from our hospital carry multiple antimicrobial resistance genes, showing a pan-resistant phenotype. The resistance phenotype and resistance genotype have a high consistency.

Objective To explore the effect and significance of age and gender on platelet and its related parameters. Methods The Beckman Coulter LH750 complete blood cell analyzer was used to measure platelet count (PLT), plateletcrit (PCT), mean platelet volume (MPV) and platelet distribution width (PDW) in 29 614 healthy adults in Shanghai Shuguang Hospital from January 1, 2019 to December 31, 2019. The results of each group were analyzed and compared. Results There were differences in PLT and PCT by gender and there were significant differences between over fifty years old and youth group (P< 0.05). PDW was closely related to age. Conclusion Gender and age could affect the results of platelet parameters to some degree, so we should establish a more detailed reference range for adults.

Analysis of the Clinical Value of Sputum Bacterial Culture Combined with NLR and Serum IL-6 Detection for AECOPD

Objective To investigate the clinical value and significance of sputum culture combined with granulocytic ratio (NLR) and interleukin-6 (IL-6) in the diagnosis of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients. Methods Retrospective analysis of 160 AECOPD patients (125 sputum culture-negative cases and 35 sputum culture-positive cases) in Bozhou people's Hospital admitted and diagnosed March 2018~ February 2020 was performed. According to whether the AECOPD process with pulmonary heart disease, respiratory failure or pulmonary hypertension or other dangerous complications, the 125 patients with negative sputum culture were divided into simple group (67 cases) and complicated group (58 cases). Another 35 COPD stable patients were selected as control group. Comparison of blood routine and serum IL-6、hs-CRP, the general clinical data and laboratory indexes were collected and analyzed. Results (1) Comparisons among three groups, there were significantly differences in NLR and IL-6 levels (P<0.05), but no statistical difference in other indicators (P>0.05).(2) NLR in pairwise comparisons among three groups:comparison of AECOPD sputum culture negative group and positive group, we found that there was no statistical difference(P>0.05), but comparison with the control group,AECOPD sputum culture-negative group and positive group showed significant differences(P<0.05).(3) IL-6 level in pairwise comparisons in three groups:comparison with AECOPD sputum culture positive-group, AECOPD sputum culture negative group and the control group showed no statistical difference(P>0.05), but there was a significant difference between AECOPD sputum culture negative group and the control group (P<0.05). (4)In the AECOPD sputum culture-negative cases, there was no statistical significance between complicated group and simple group. (5)The ROC curve analysis of each inflammatory index factor in the sputum culturenegative group of AECOPD patients showed that the area of NLR and IL-6 under the curve of sputum culture negative patients was 0.720, 0.689. Conclusion The results of sputum bacteria culture combined with the expression level of NLR and IL-6 can assist in the diagnosis of AECOPD patients, especially for AECOPD patients without specific indicators in sputum bacteria culture negative.
