血清miRNAs作为潜在肝癌诊断标志物的临床价值研究*

Clinical Value Study of Serum MiRNAs as Potential Diagnostic Markers for Hepatocellular Carcinoma

摘要: 目的 探讨7种miRNA,简称miRNA7(包括miR-21,miR-26a,miR-27a,miR-122,miR-192,miR-223和miR-801)在肝癌患者中的表达水平,比较其与甲胎蛋白(AFP)、癌胚抗原(CEA)联合检测在早期肝癌中的诊断效能。方法 数据库ENCORI包含了50例肝正常组织和370例肝癌组织的相关数据信息,检索miRNA7在不同肝组织间表达水平的差异;随后收集我院诊治的70例非癌性患者作为肝良性组,75例肝癌患者作为肝恶性组,32例健康体检者作为对照组,比较三组血清miRNA7、AFP和CEA表达水平、阳性率,分析以上指标单独及联合检测对肝癌早期诊断的价值。结果 ENCORI数据库显示,相比肝正常组织,肝癌组织中miR-21,miR-26a,miR-27a,miR-122,miR-192和miR-223均显著异常表达。在177例受试者中,相比于对照组和肝良性组,肝恶性组血清miRNA7的总相对定量值Logit、AFP和CEA表达水平及检测阳性率均显著升高,差异均有统计学意义(P<0.05)。ROC曲线显示,肝恶性组和对照组的血清miRNA7、AFP和CEA联合检测的AUC值(0.97)明显高于三者单独检测,差异均有统计学意义(P<0.05),说明三者联合检测有利于提高早期肝癌的诊断效能。同时,肝恶性组和良性组间血清miRNA7、AFP和CEA联合检测的AUC值(0.92)也均高于其单独检测,三者联合检测的AUC值与单独检测AFP、CEA差异有统计学意义(P<0.05),但与miRNA7差异无统计学意义(P>0.05)。结论 血清miRNA7是肝癌早期诊断的有效肿瘤标志物,与AFP、CEA联合检测可提高早期肝癌的临床诊断效能。

Abstract: Objective To investigate the levels of 7 kinds of miRNA,miRNA7(including miR-21, miR-26a, miR-27a,miR-122,miR-192,miR-223 and miR-801)in serum of patients with hepatoma. And comparing the diagnostic efficacy of miRNA7 with alpha-fetoprotein(AFP) and carcinoembryonic antigen(CEA)in early hepatoma. Methods ENCORI database included 50 normal liver tissues and 370 liver cancer tissues,and the difference of miRNA7 expression levels among different liver tissues was searched. Subsequently,32 healthy subjects were collected as the control group,70 non-cancer patients as the benign liver group, and 75 liver cancer patients as the malignant liver group. The expression levels and positive rates of miRNA7,AFP and CEA in serum of the three groups were compared, as well as the value of single detection and combined detection in the early diagnosis of liver cancer. Results ENCORI database showed that miR-21, miR-26a,miR-27a,miR-122,miR-192 and miR-223 were significantly abnormal in liver cancer tissues compared with normal liver tissues. In 177 subjects,the total relative quantitative value of serum miRNA7 Logit,AFP and CEA expression level and positive detection rate in hepatic malignant group were significantly increased,with compared to healthy control group and benign liver group, the differences were statistically significant(P<0.05). ROC curve of liver malignant group and control group showed that the AUC value(0.97)of serum miRNA7, AFP and CEA combined detection was significantly higher than that of serum miRNA7,AFP and CEA alone,and the difference was statistically significant(P<0.05), indicating that the combined detection of those three is beneficial to improve the diagnostic efficiency of early liver cancer. Meanwhile,the ROC curve between malignant and benign groups was analyzed,and the AUC value(0.92)of serum miRNA7,AFP and CEA combined detection was also higher than that alone. There were statistically significant differences between their combined detection with AFP and CEA alone(P<0.05),but there were no significant difference with miRNA7 alone(P>0.05). Conclusion Serum miRNA7 is an effective tumor marker for early diagnosis of liver cancer,and its combination with AFP,CEA can improve the clinical diagnostic efficiency of early liver cancer.
