衡阳地区无偿献血者核酸检测结果分析*

摘要: 目的 评估衡阳地区无偿献血者应用核酸检测技术筛查血液,降低输血相关感染风险的效果。方法 采用2种ELISA试剂对2015年1月~2016年12月的84 622例无偿献血者血液标本进行检测,大部分经检测无反应性/待查(单试剂阳性或者灰区)的标本计77 610例,采用COBASs 201核酸检测系统进行HBV/HCV/HIV三项核酸检测;对核酸HBV反应性标本做乙肝两对半补充实验。结果 经核酸检测的77 610例献血者血液标本,反应性比率为0.278% (217/77 610),其中检出HBV DNA 216例,HCV RNA 0例,HIV RNA 1例,低于血清学HBsAg、HCVAb、HIVAb/Ag三项总的检测阳性率1.81% (1 533/84 622)。核酸反应性标本的HBcAb+/HBsAg-为41.15%(86/209),远高于一般人群中HBsAg阴性人群的HBsAb检出率17.1%(22/129)。结论 目前衡阳地区输血传播疾病的残余风险主要是输血感染HBV。核酸检测技术的应用能有效保障血液安全,而建立长期的低危献血者队伍,能从源头确保血液安全。

Abstract: Objective To research the effect of risk of blood transfusion in voluntary blood donors through investigating the results of the nucleic acid test of Hengyang.Methods From January 2015 to December 2016,84622 blood samples were collected from the donors and detected bytwo rounds of ELISA. Then,77610 blood samples,whether or not they were tested reactive by one reagent of ELISA,would be tested by NAT.The markers of HBV were detected in blood samples which had HBV-DNA . Results 217 blood samples out of 77610 were detected positive (at a rate of 0. 278%,216 blood samples were HBV-DNA positive,0 was HCV-RNA,1 was HIV-RNA.The NAT detection rate was less than that of ELISA (1.81%,1 533/84 622). 86 HBV-DNA positive samples were detected by ELISA with HBcAb in 209 (41.15%) and significantly higher than the detection rate of HBcAb in the common population without HBSAg. Conclusion Resultssuggested that the implementation of NAT had effectively improved the blood safety and decreased the risk of blood transfusion of HBV in Hengyang. Blood banks shall consolidate and develop the low risk blood donors team to ensure the blood safety from source.
