The Treatment Measures and Clinical Application of Plasma Exchange at Bedside

Objective To establish the treatment technique of therapeutic plasma exchange (TPE) by domestic blood separator (NGL-XCF3000) at bedside for the clinical application to treatments in 154 patients. Methods Medical records of 154 patients,who received TPE treatments in the hospital between 2010 and 2016,were collected and retrospectively analyzed. The TPE at bedside was established,and the clinical feasibility was studied. Results The overall efficacy rate of the treatment technique was 86.4%. The results of TPE clinical applications showed a total effective rate of 83.8% in 74 cases of Guillain-Barre syndrome. An improvement of dyskinesia was seen in the patients when the TPE was used combined with glucocorticoids (P<0. 05). Similarly,the effective rates of 90 per cent and 86.7 per cent were achieved in 50 cases of myasthenia gravis and 30 cases of acute liver failure,respectively. The overall incidence of adverse effects was found to be 8.4% in 154 cases. Conclusion The domestic products are safe and effective for the therapeutic plasma exchange at bedside and worth using clinically.

Efficacy of Therapeutic Plasma Exchange in Treatment of Acute Hyperlipidemic Pancreatitis

Objective To investigate the therapeutic effect of plasmapheresis in treatment of acute hyperlipidemic pancreatitis (AHP). Methods A total of 16 AHP patients in the hospital between 2015 and 2016 were collected and treated with therapeutic plasmapheresis. The levels of triglyceride (TG),amylase (AMY) and lipase (LPS) before and after treatments were examined. Results Sixteen AHP patients who received plasmapheresis with the average of 3 treatments,2000 mL for each,showed significant decreases in TG (11.96±5.66 mmol/L vs 3.56±1.06 mmol/L),AMY (259.14±173.40 IU/L vs 41.71 ±25.25 IU/L),and LPS (341.92±257.10 IU/L vs 43.5±19.54 IU/L) (P<0.05). Conclusion Therapeutic plasma exchange is an effective method for treatment of acute hyperlipidemic pancreatitis.

A Comparison of Plasma Exchange Performance between Two Blood Cell Separators

Objective To compare the efficacy of two blood cell separator in reduction of blood group antibody titers in renal transplants with incompatibility of blood groups. Methods Plasma exchange was performed on 23 patients with ABO incompatible renal transplant recipients between 2015 and 2017 by using the CS-3000 and COM.TEC blood cell separators. Results CS-3000 gave rise to the replace of 1.3±0.5 plasma volume and COM.TEC resulted in the replace 0.9±0.2 plasma volume for a half reduction of the antibody titers. The procedure time to exchange one plasma volume was 95.2±10.4 min with the CS-3000 and 75±11.2 min with the COM.TEC (P<0.001). The plasma removal efficiency (PRE) was 39.7±4.9% with the CS-3000 and 48.2±3.7% with the COM.TEC (P<0.001). In comparison with the blood components before plasma exchange,the platelet counts of patients with the CS-3000 blood cell separator replacement obviously decreased by an average of 25.42% while those of patients with the COM.TEC reduced by 12.56%. Conclusion Compared with the CS-3000,the COM.TEC might improve PRE and have short procedure time and slight losses of platelet counts for exchange of one plasma volume. Additionally,the COM.TEC is suitable for kidney transplant recipients with high initial antibody titers and low number of platelets.

The Safety of Therapeutic Plasmapheresis in Patients with Critical Diseases

Objective To observe the safety of therapeutic plasma exchanges (TPE) in patients with critical diseases such as hematologic,neurologic,and metabolic disorders. Method Data of therapeutic plasma exchanges were collected in patients from Oct 2012 to Dec 2017. Detailed information of adverse events were recorded. Blood cell loss,hemolysis and changes of antibody titers were measured. Result Sixty-eight patients received 262 TPE treatments. Among them,33.8 percent of the patients were seen to have TPE related complications,with 16.8 percent of complication occurrence. After corresponding treatments,the adverse reactions were remitted immediately and follow-up treatments were completed. Blood loss of each TPE session was less than 1% of the patient’s total blood volume. No obvious hemolysis was noted in waste plasma bag. The maximum decrease in HBsAb was achieved on the final day of TPE sessions (70-80% reduction). A slow return to baseline for HBsAb was observed. Conclusion The majority of complications were mild and manageable. The TPE may not cause obvious blood loss and hemolysis. HBsAb was detectable during the treatment. The TPE is a safe and well tolerated treatment option in several illnesses.

Analysis of Influencing Factors in Collection of Peripheral Hematopoietic Stem Cells in Children

Objective To observe the effects of gender,age,peripheral blood WBC and MNC count on the collection of peripheral blood hematopoietic stem cells in children. Methods From January 2016 to January 2018,28 cases of peripheral blood hematopoietic stem cells collected in the hospital,ranging in age from 2 to 10 years and the median age was 10. Among 31 collections,7 autologous cases were given chemotherapy combined with granulocyte colony stimulating factor (GCSF) for mobilization,and 24 allogenic cases received GCSF mobilization,and 4-5 days later,peripheral blood hematopoietic stem cells were collected. The mean single circulating blood volume was 7 593±2 248 mL and the single product volume was 126 ±42 mL. Stem cell products were subjected to blood routine counting and flow cytometry to determine the number of CD34+ cells. Results Achievements of monocytes (MNCs) were uncorrelated to gender and age (P=0.44; P=0.81) in single collection but associated with counts of peripheral WBCs and MNCs (P=0.03; P=0.01). Moreover,CD34+cell counts were found to be not associated with gender,age and peripheral WBCs counts (P=0.15; P=0.79; P=0.66),but with pre-harvest counts of MNCs (P=0.02). Twenty four patients had MNCs>3.75×109/L before collection. The number of MNCs in single-collected stem cell products was 9.88±3.16×108 / kg and the number of CD34+cells was 7.51±4.74×106/Kg. Conclusion The number of MNCs in peripheral blood is an important factor affecting the collection efficiency of peripheral blood hematopoietic stem cells in pediatrics. The number of peripheral blood MNCs>3.75×109/L after mobilization would benefit for evaluation of the effect of hematopoietic stem cell mobilization.

Objective To explore the efficacy of "Internet +" mode in recruitments and appointments of blood donations so as to improve the process management and efficiency of blood component donation. Methods Blood donors were recruited through the WeChat Public Account from 2014 to 2016 and the results were compared with those recruited by traditional paths such as face-to-face,telephone appointments and company introduction. Finally,the performance of platelet donations was analyzed. Results The number of performance for platelet donations was yearly increased from 53.68% in 2014,64.88% in 2015 to 66.87% in 2016,while it gradually decreased from 46.32% in 2014,35.12% in 2015 and 33.13% in 2016 when recruited by the traditional means. Conclusion “Internet +” may greatly improve the efficiency of recruitments and appointments for platelet donations compared with traditional means.

Objective To investigate the serological characteristics of one case of B(A) blood type identified by DNA sequencing. Methods The ABO blood phenotypes were determined by routine serological methods,and the ABO genotype was determined by direct sequencing and cloning sequencing. Results The serological results of positive and reverse ABO blood typing were inconsistent.The positive serological result presented AB,while the A antigen was relatively weak and the H antigen was stronger than that of the normal type B. The reverse serological detection showed the existence of weak anti-A1. Complete regions of sixth and seventh exons of the ABO gene were amplified .The sequencing result showed that allele O02 was found in the sixth exon,and 640 A>G mutation was showed in the seventh exon. It showed an allele of B(A)04 and genotype of B(A)04 /O02. Conclusion The blood type was identified as B(A),and further genotype was identified as a rare B(A)04 /O02.

Objective To investigate the occult hepatitis virus (OBI)infection in blood donors in Shaoguan. Methods All samples were screened for HBsAg,anti-HCV,and HIV antibodies by EIAs. Meanwhile,HBV DNA,HCV and HIV-1 RNA were tested by NAT(nucleic acid test).Viral loads of HBsAg-/HBV DNA+ samples were quantified by QPCR,and BCP/PC and S were amplified by Nest-PCR .HBV genotype were detected and analyzed. Results 19 samples with normal ALT were detected as HBsAg-/HBV DNA+ in 18030 blood donators,NAT yielded 1:949. All samples were defined as normal in ALT test. 16 were classified as OBIs with a relevance ratio of 1:1126,11 samples were genotype B and 2 samples were genotype C,5 samples were undetectable. Conclusion NAT screening can improve transfusion safety.

Objective To uncover the situation of syphilis infection among voluntary blood donors in Fuyang so as to provide the information for prevention from the blood-transmission of syphilis. Methods The screening results of syphilis antibodies in 156 740 unpaid blood donors were analyzed from January 2014 to December ~2016 retrospectively. Results The serological prevalence of syphilis was found to be 0.45% when antibodies against syphilis tested,with a yearly decrease of the antibody positive rates. The frequency of syphilis infection was higher in female than in male but no remarkable difference was seen in age. Conclusion It is essential to strengthen the organization and education to and consultation with the unpaid blood donors as well as to use the reagents with high quality,specificity and sensitivity for further reduction of the risk of syphilis infection.

Objective Our aim is to investigate the relationship between periodontitis and renal function. Methods A total of 90 patients with periodontitis treated in our hospital were selected as case group and 90 healthy persons of periodontal examination were selected as the control group. The levels of serum BUN,CR,eGFR,CysC and SOD were compared between the two groups to study the relationship between periodontitis and renal function. Results The serum levels of BUN and CysC in the case group were higher than those in the control group,the levels of eGFR and SOD were lower than those in control group(P<0.05). No significant difference was found in the level of CR between the two groups (P>0.05). Conclusion Patients with periodontitis have obvious abnormal renal function related indexes,and periodontitis may be one of the risk factors of kidney disease.

Current Situation of Serum AMH Level of Healthy Women in Baoan District and its Application Value in PCOS and POF Treatment

Objective To investigate the current situation of anti-mullerian hormone (AMH) in healthy women of healthy childbearing age in Baoan district Shenzhen city,and to explore its application value in PCOS and POF treatment. Method We collected health check-up healthy women of childbearing age 1 200 cases as control group in March 2016 to May 2017 in Shenzhen baoan district maternity and child care,and selected polycystic ovarian syndrome(PCOS)patients 85 cases and premature ovarian failure(POF)patient 67 cases at the same period,detected the serum AMH level of Control group,patients with PCOS and POF group at before the treatment and after 3-months treatment respectively,and analyzed the testing results. Results The serum AMH level of 1 200 cases of healthy women of childbearing age was 4.97±1.64 ng/ml,19~25 years old group was 5.41±1.38 ng/ml,significantly higher than 31~35 and 36~41 years old group,was statistically significant difference (t=3.092~5.126,P<0.05),compared with 26~30 years old group (5.07±1.52 ng/ml) there was no statistically significant difference (t=0.906,P>0.906). Before the treatment,the serum AMH level of patients with PCOS was 9.76±3.82 ng/ml,significantly higher than the control group,POF and wait POF group,difference was statistically significant (t=13.294~13.294,P<0.01~0.001); the serum AMH level of POF patients was 0.94±0.36 ng/ml,significantly lower than the control group and wait POF group,difference was statistically significant (t=2.915~2.915,P<0.05); the serum AMH level of wait patients was 2.37±0.82 ng/ml,significantly lower than the control group,the difference was statistically significant (t=8.604,P<0.05). After treatment,the serum AMH level of PCOS,POF and wait POF patients were 5.29±1.62 ng/ml,4.25±1.24ng/ml and 4.51±1.50 ng/ml,and the difference was statistically significant between before treatment (t=7.672~11.057,P<0.05),compared with the control group there were no statistically significant differences (t=0.790~1.527,P>0.05). Conclusion The serum AMH levels of PCOS patients were significantly higher in before treatment,and the serum levels of POF and wait POF patients were significantly reduced,while the serum AMH levels of PCOS,POF and wait POF patients were all restored to normal levels after treatment. Therefore,AMH has certain application value in the diagnosis and treatment evaluation of women PCOS and POF disease.

Objective To investigate the expression of inflammatory biomarkers in patients with recurrent miscarriage. Methods Using blood cell analyzer examination of peripheral blood cell analysis in healthy young women group,normal pregnancy group,habitual abortion patients,calculating of inflammatory biomarkers NLR,PLR and LMR,immunoturbidimetricassay was used to detect C–reactionprotein,meanwhile,Th1/Th2cytokines levels in serum were measured by flow cytometry.Results Compared with healthy young women and normal pregnancy,the NLR、LMR、PLRlevels in patients with habitual abortion were respectivelysignificant differences(P<0.05),the CRP levels in patients with habitual abortion were respectively significant differences(P<0.05).The IL-4 levels in patients with habitual abortion were significantly lower than other two,the IL-2 levels in patients with habitual abortion were higher than other two,there were significant differences(P<0.05). Conclusion Inflammatory biomarkers participate in the development of habitual abortion.

Effect of Neonatal Transitional Hypoglycemia on Neurodevelopmental Outcomes at 18 Months Age

Objective To investigate the impact of neonatal transitional hypoglycemia on neurodevelopment in the infants at 18-months age. Methods Fifty-eight hospitalized infants with hypoglycemia were enrolled within 48 h after birth. The infants were given standardized management,aEEG detection,MRI scan,and assessed by using the Bayley Infant Development Scale (BSID) to 18 months after discharge. Results The median of hypoglycemia occurrence time after birth is 3.86 h(0.5~44 h),with the duration of 2.03±0.67 h,and venous blood glucose concentration was 1.62±0.37mmol/L. The first aEEG findings included disappeared sleep-wake cycles in 2 cases,immatures in 7 cases,and convulsions in 2 cases. The repeated aEEG examination revealed immature sleep-wake cycle in 2 cases. Brain MRI scans showed 7 abnormalities of 45 cases. BSID assessment of the infants at 18 months age demonstrated 93.91±6.35 of MDI and 95.86±7.27 of PDI. Spearman correlation analysis between blood glucose and MDI,PDI showed no obvious correlation (r:0.061,0.656,P>0.05). The scatter diagram of each point presented mess disorder.Conclusions Developmental quotients of neonatal transitional hypoglycemia infants were normal at 18 months and were not associated with lowed blood glucose levels if they were received standardized blood glucose management after born. Long-term prognosis need further following-up.

Association of Colchicine and CA72-4 and Pepsinogen Levels in Patients With Gout

Objective To investigate the correlation between tumor markers CA72-4 and pepsin levels and colchicine treatment in gout patients. Methods From January 2016 to July 2017 in 64 cases of gout in Department of Endocrinology and Rheumatology in our hospital,the patients were divided into two group according to whether or not colchicine was taken in the first two weeks before admission. The patient's general data were collected including gender,age,duration of disease,and levels of CA72-4,and pepsin. In order to exclude gastrointestinal and other tumors,electronic gastroscopy,electronic colonoscopy,abdominal color Doppler ultrasound examination were carried out. Results ①64 patients had history of gout and no tumor diseases. ②There was no significant difference between the two groups in sex,age,course of disease,ESR,UA,CRP and PCT. ③CA72-4 group was significantly higher than non - taking group (P<0.05). After two weeks' withdrawal,the level of CA72-4 decreased significantly,the difference was statistically significant (P<0.05). ④The difference of PGI between the two groups during the period of medication and after withdrawal was statistically significant,but there was no statistical difference between PGII and PGI/PGII. Conclusion Colchicine treatment in patients with gout may be associated with elevated CA72-4. Check CA72-4 after stopping medication to exclude the impact of drugs on the test results; Colchicine treatment of gout and PGI have a certainly relevance.

Changes of Serum Procalcitonin Levels in Patients with Early Infection after Rheumatoid Arthritis Replacement

Objective To investigate the changes of serum procalcitonin levels in patients with early infection after rheumatoid arthritis replacement. Methods a retrospective analysis of 20 cases of rheumatoid arthritis after replacement of early infection patients as research group A,200 cases of replacement rheumatoid arthritis without postoperative infection in patients with as the study group B,40 healthy subjects served as study group C were carried out in our hospital from February 2016 to February 2017 were. The erythrocyte sedimentation rate,C-reactive protein,procalcitonin,WBC count,specificity,sensitivity,negative predictive value and positive predictive value of the three groups were compared and analyzed. Results 1.Sedimentation rate,C-reactive protein and procalcitonin,replacement of rheumatoid arthritis patients the level of white blood cell count and study group B after infection were compared in patients with early infection of red blood cell group A rheumatoid arthritis after arthroplasty is significantly higher,P<0.05 was statistically significant in patients with red; cells not infected with rheumatoid arthritis with replacement of group B after sedimentation rate,C-reactive protein and procalcitonin,white blood cell count and the level of C subjects,significantly higher,P<0.05 was statistically significant. 2.The sensitivity of the level of PCT and ESR and the sensitivity of WBC,CRP and PCT in patients with early infection after rheumatoid arthritis replacement were significantly higher,and the positive rate of PCT positive was the highest. 3.The AUC of PCT is 0.752,the truncation value is 15.70ng/mL,the standard error is 0.061,and the 95%CI range is from 0.633 to 0.982. Conclusion Serum procalcitonin levels in patients with early infection after rheumatoid arthritis replacement can be used as diagnostic criteria. The cut-off value is 10.50ng/ml,which can be used as a reference for clinical diagnosis.

Objective Analysis of the effect of United hemodialysis and hemoperfusion applied to hemodialysis patients in maintenance therapy on serum parathyroid hormone. Methods 74 cases in our hospital hemodialysis patients met the inclusion criteria were randomly divided into A,B group,37 cases in each group. A group using a simple blood dialysis treatment,group B hemoperfusion combined with hemodialysis. Observed levels of inflammatory cytokines before and after treatment,serum parathyroid hormone levels,serum parathyroid hormone clearance and improvement of clinical symptoms,and expand the comparative analysis. Results (1) levels of inflammatory factors:group B after treatment hs-CRP,TNF-α,IL-6 compared with group A,the difference was statistically significant (P<0.05). (2) serum parathyroid hormones:parathyroid hormone index after treatment in group B compared with group A,the difference was statistically significant (P<0.05); after treatment in group B serum parathyroid hormone clearance rate is higher than that in group A,the difference was statistically significant (P<0.05). (3) the rate of clinical improvement:group B clinical symptoms improvement rate than A group,the difference was statistically significant (P<0.05). Conclusion For patients undergoing maintenance hemodialysis regular hemodialysis combined with hemoperfusion therapy,can significantly reduce levels of inflammatory cytokines and serum parathyroid hormone levels in patients with increased serum parathyroid hormone clearance rate,improve clinical symptoms.

Staphylococcus Aureus Genotyping and Multi-drug Resistance in Pediatric Respiratory Tract Infections

Objective To explore Staphylococcus aureus genetic diversity and the mechanisms of multi-drug resistance in respiratory tract infections of children. Methods A total of 72 strains of S. aureus were isolated from fresh sputa or throat swab specimens in children with respiratory tract infections. The drug sensitivity was detected by VITEK full automatic microbial identification system,the β-lactam,amino glycosides,tetracycline. Large ring lactone drug resistance gene of the isolates was detected by PCR,and the drug resistance gene frequency of MRSA was calculated. Results Thirty four MRSA positive strains were screened out from 72 strains of S. aureus,34 strains of mecA genes were detected in 72 strains of S. aureus,with a prevalence of 47.2%. The frequency of MSSA resistant to amoxicillin,erythromycin,gentamicin,minocycline was significantly lower than MRSA (P<0.05). Bacterial sensitivity to non β-lactam and resistance to 1-2 type non β-lactam in MRSA were remarkably lower than MSSA,MRSA multi-drug resistant rate was significantly higher than MSSA (P<0.05).The tetM,aac(6')/ ph(2''),ermA,tetK detection rate in MRSA strains was significantly higher than that of MSSA. The ermC detection rate of MSSA strain was significantly higher than that of MRSA (P<0.05),and there was no significant difference in (3 ') - III gene detection rate between MSSA and MRSA (P>0.05). The frequencies of non-resistance gene or1-2 resistance genes in MRSA were lower than those of MSSA. Conclusion The main genotypes associated with resistance of S. aureus are mecA,tetM,aac(6')/ ph(2''),ermA,and tetK in respiratory tract infections of children,which may constitute the main mechanism of multiple drug resistance.

As a new technology of compatibility detection of blood transfusion,electronic blood cross matching has been used in some developed countries for many years,and the feasibility and safety of applications related have also been studied in recent years. In this paper,through the brief understanding of the research of the electronic cross matching technology,the security problems of the domestic technology of the application are expounded. Even if some domestic hospitals already have met conditions to carry out electronic matching,but because of the lack of standards,some key technologies to be further standardized and clear,still can not guarantee that the new technology in the safety of clinical application,it attracted enough attention. Electronic identification,such as the identity of the two patients,the blood type antibody screening reagent red blood cell antigen spectrum of the design and construction of transfusion quality management information system,access system,required to be strictly regulated,suitable for clinical application of domestic electronic matching technology standard,can guarantee the safety of this technology in clinical application.

Analytical Performance of Thyroid Hormone Using Global External Quality Assurance System

Objective To analyze retrospectively the performance of thyroid hormone tests in the Bio-Rad external quality assurance system (EQAS) so as for continuous improvements of laboratory performance. Methods Data of total triiodothyronine (TT3),total thyroxine (TT4),free T3 (FT3),Free T4 (FT4) and thyroid stimulating hormone (TSH) from 12 samples in the ninth cycle were analyzed by Bio-Rad EQAS. And Z-score,mean Z-score and L-J distribution histogram at different concentration levels in the peer and method groups were evaluated with Beckman Coulter DXI 800 analytical system. Continual improvements were made for reliable performance. Results Samples No.6 of FT3,No.6 of FT4 and No. 3 of TT4 were warned due to their Z-scores between 2 and 3. And performance of sample No.5 of FT3 could not be accepted because its Z-scores were above 3. The average Z scores of TT3,TT4,FT3,FT4 and TSH in the 9th cycle were below 2 although that of FT3 was inferior to 97% of participants. Our laboratory performance was better in the 10th cycle than in the 9th cycle after continual improvements. Conclusion Further improvements would be necessary for the quality assurance of thyroid hormone analysis in personnel,environment,instrument detection system and sample processing.
