Clinical Significance of Platelet Antibody Detection——Based on Study of 2 338 Case from10 Institutions in China

Objective To explore the relationship between platelet antibodies and disease symptom, weinvestigatedthe clinical datafromthe detectionof plateletantibodies in 10 institutions of China. Methods The demographic, clinical and laboratory data of patients who were detectedin platelet antibodies in 10 institutions across the country from July 1, 2018 to June 30, 2019 were collected and statistically analyzed for platelet transfusion refractory. Relationship between Fetal and neonatal alloimmune thrombocytopenia (FNAIT), abortion and stillbirth, and immune thrombocytopenia. Results Among the 2 338 patients, the positive rate of platelet antibody in patients with platelet transfusion refractory, FNAIT patients, abortion and stillbirth patients, and immune thrombocytopenia was higher than that in patients without the above disease (control group). Conclusion Platelet antibody detection is important in the prevention and diagnosis of platelet transfusion refractory, FNAIT, abortion and stillbirth, and immune thrombocytopenia. It should be included in the routine laboratory examination of the above diseases.

A Retrospective Analysis of Platelet Inhibition Rate in AA and ADP Pathways Detected by Thrombus Elastography

Objective To retrospectively analyze the influencing factors of arachidonic acid (AA) and adenosine diphosphate (ADP) inhibition rates and the efficacy of different antiplatelet drugs. Methods A total of 114 patients with platelet inhibition rates detected in the hospital were selected from March 2015 to December 2018 . The inhibition rates of AA and ADP were divided into several groups according to the antiplatelet treatment strategies. Results The ADP inhibition rate,rather than the AA inhibition rate(P>0.05),was negatively correlated with age (P<0.01) and positively with triglyceride (TG) test results (P<0.01).Significant differences of ADP inhibition rates between groups of aspirin and aspirin+Greg los(P< 0.01) and between groups of clopidogrel and aspirin+Greg los(P<0.05) were noted.Similarly,remarkable differences of AA inhibition rates between groups of aspirin and clopidogrel(P<0.01),clopidogrel and aspirin+Greg los(P<0.01),and clopidogrel and aspirin+clopidogrel(P<0.05) were seen. Conclusion A comprehensive analysis of age and TG level should be taken into account in terms of the ADP inhibitory rate detection.Greg los treatment is effective based on the ADP inhibition rate measurement while aspirin therapy is promising based on AA inhibition.

The Qualitative Observation of Leukocyte-Free Red Blood Cells in Additive Solution during Storage

Objective To investigate the processing feasibility and quality after the storage of the leukocytes removed red blood cells in additive solution. Methods One bag of ,leukocyte removed red blood cells in additive solution was divided into six equal volumes. One of them was stored in refrigerator at 4±2℃, without washing as control group whereas the others were washed by saline or MAP, suspended in saline, MAP or ACD. Saline washed-saline suspended group, saline washed-MAP suspended group, saline washed -ACD suspended group, MAP washed -MAP suspended group, and MAP washed -ACD suspended group were taken for experiments. Samples were subjected to different preservation time (0~35 d) after preparation. The variance analysis was conducted according to the factorial design of repeated measurement of different groups and time intervals. Results HGB content, supernatant protein content, residue leukocytes and sterility test were all in line with the national standards. The K+ concentration, pH value and FHb content were statistically different at each “time”and “group”. Conclusion The leukocyte-removed erythrocytes can be washed and suspended in added liquid for a long time with viability. The suitable washed RBCs products are recommended in clinical transfusion. The washed red cells suspended in saline within 24 hours or in additive solution stored within three days would be appropriate.

Clinical Significance of C1q-HLA Antibody in Patient Refractory to Platelet Transfusion

Objective The specificity of HLA and HPA antibodies as well as C1q-HLA antibodies in patients with platelet transfusion refractoriness (PTR) was analyzed to determine the clinical significance of C1q-HLA antibody for PTR patients. Methods LABScreen® Single Antigen (LSA) kit and C1qScreen™ kit were used to detect the specimens from HLA antibody-positive PTR patient to analyze the specificity of HLA and HPA antibodies, and whether these HLA antibodies could combine with complement component C1q (i.e. C1q-HLA antibodies). Results In 40 patients, 10 patients (25%) had HPA antibodies which were mainly against glycoprotein GP IIb/IIIa. In 39 cases of HLA-LSA-Positive and 28 C1q-positive patients, the percentage of panel reactive antibody (PRA%) and Max mean fluorescence intensity (MFI) of LSA-positive patients were significantly higher than those of C1q-HLA-positive patients. There was no significant difference in the CCI value between C1q-HLA-positive and -negative patients after random platelet transfusion. Different status of HLA antibodies had no significant effect on finding compatible cross-matched platelets for PTR patients. Conclusion The results of this study showed that there was no direct correlation between C1q-HLA antibodies and clinical result of platelet transfusion. The occurrence of C1q-HLA antibodies may not increase the difficulty of finding compatible cross-matched platelets for PTR patients.

Study on the Application of Advanced Autologous Apheresis in Idiopathic Scoliosis

Objective Toevaluate the clinical effect of advanced autologous apheresis (AAA) in avoiding allogeneic blood transfusion in idiopathic scoliosis surgery. Methods We reviewed 32 idiopathic scoliosis patients who underwent elective surgery at a single hospital from July 2017 to July 2018. The patients were divided into AAA group and control group. We compared general information, surgical characteristics, autologous red blood cells (RBC) collecting amount, allogeneic RBC and plasma transfusion volume, laboratory data including RBC count, hemoglobin (Hb) level, hemotocrit (HCT), platelet count (PLT) in preoperative 3 days, operating time, postoperative 1 day and 3 days. Results Two cohorts had no differences in general information, surgical characteristics and preoperative levels of RBC, Hb, HCT and PLT (P>0.05). Compared to control group, autologous blood collecting amount in AAA group was much higher(P<0.05), and allogeneic RBC transfusion volume was significantly reduced(P<0.05). After autologous blood collecting, RBC, Hb and HCT levels of patients were lower than before(P<0.05), and no decline in PLT levels(P>0.05). Two cohorts had no differences in postoperative in RBC, Hb, HCT and PLT levels(P>0.05). Conclusion AAA can reduce allogeneic transfusion in idiopathic scoliosis patients.

The Dilemma and Countermeasures of Human Resource Allocation in Blood Collection and Supply Institutions

Objective To understand the current situation of human resources allocation in blood collection services, and to provide decision-making basis for optimizing human resources allocation. Methods The questionnaire of human resources in blood collection services was designed, and the data were obtained by questionnaire survey and on-site visits. The data of blood collection and supply, the number of staff, educational background and professional title were compared with those of the whole country. Results There were 1 blood center and 11 central blood stations in Zhejiang Province. The average number of staff needed to collect one thousand units of blood was 1.05 and 1.57, respectively, which was significantly lower than the national average of 1.69 and 2.57. Among the staff of blood collection services in Zhejiang Province, 0.09% were doctors, 5.76% were masters, 66.33% were undergraduates, 16.83% were specialists, 10.98% were secondary schools or below. The distribution of academic qualifications was concentrated in undergraduates and specialties, and the shortage of doctoral qualifications was serious. The proportion of senior, middle and junior titles in blood collection and supply institutions in Zhejiang Province was 14.27%, 30.01% and 42.09% respectively, while the others were 13.63%. The distribution of professional titles was concentrated in junior and middle levels. In recent three years, 283 staff were recruited into blood collection services of Zhejiang Province has been, and 230 quit their jobs in blood collection services, among which, the rates of doctors, nurses and medical technicians were 10.49%, 33.14% and 16.52%, respectively. Conclusion The per capita business volume of blood collection services in Zhejiang province was significantly higher than the national average, which has the characteristics of large personnel mobility and low educational level of talent team. It is suggested to formulate detailed personnel allocation standards and reasonable performance allocation measures for blood collection services in order to attract excellent talents and maintain the stability of the talent team.

Detection of Antibodies Against Toxoplasma Gondii in the Blood Donors and Its Risk Analysis of Residual Blood Transfusion

Objective To observe the antibody prevalence of Toxoplasma gondii in blood donors and analyze the residual risk of blood transfusion in Tangshan City. Methods A total of 308,024 blood samples were tested for IgG and IgM antibodies to T. gondii. The distribution characteristics, risk factors and residual risk of blood transfusion were analyzed. Results There were 3 453 cases (1.14%) of blood donors who were found to be positive for Toxoplasma antibodies, and the frequency trends to be increasing (from 0.92% in 2013 to 1.30% in 2018) (χ2=32.449, P<0.05); the positive rates were high in those with undercooked meat consumption, frequent contact to animals and repetitive blood donations(3.78%, 2.85% and 2.14%, respectively). The total risk of Toxoplasma transfusion residue in the blood donors was 9.39×10-6, and that in those with raw/undercooked food consumption (37.33×10-6) was high (5.57×10-6). The risk of blood transfusion residue in those with frequent contact to animals (40.06×10-6) and repeated donations (14.30×10-6) was elevated compared with the other donors (P<0.05). Conclusion The antibody prevalence of Toxoplasma gondii in blood donors seems to be increasing. The high risk of T. gondii residue in blood donors with raw/undercooked foods and/or frequent contact to animals may be a potential threat to blood safety .

Syphilis Confirmatory Testing and Syphilis Infection Status Analysis of the Blood Donors on the Unqualified Results of Anti-TP Antibody Screening

Objective Syphilis confirmatory testing and syphilis infection status analysis of the blood donors on the unqualified results of anti-TP antibody screening, in order to provide the reference of setting the scientific and reasonable grey area and informing the test results of the blood donors and shielding the blood donors. Methods From 24 March to 2 October in 2016, 290 blood samples of the blood donors on the unqualified results of screening anti-TP antibody were collected in Anhui blood center. Syphilis confirmation was performed by TPPA and the agglutination titer of the Non-treponema pallidum antibody was detected by TRUST. Results 290 unqualified samples of anti-TP antibody screening were confirmed by TPPA, there were 166 TPPA positive samples and 124 TPPA negative samples. There were 50 TRUST positive samples among 166 TPPA positive samples. The positive rate of anti-TP ELISA screening reagent 1 and reagen 2 showed no significant difference (P>0.05). There wasn't TPPA positive samples among 107 samples with single reagent positive and double reagent grey area for anti-TP antibody ELISA screening. Among 290 unqualified samples of anti-TP antibody screening, there was no significant difference (P>0.05) between male TPPA positive rate (57.31%) and female TPPA positive rate (57.14%), there was significant difference that the married persons TPPA positive rate (68.78%) was higher than the unmarried persons its (35.64%) (P<0.05), the married males TPPA positive rate (70.18%) was higher significantly than the unmarried males (31.58%) (P<0.05), the married females TPPA positive rate (66.67%) was higher significantly than the unmarried females TPPA positive rate (40.91%) (P<0.01), the TPPA positive rate of the once blood donors (61.07%) was higher significantly than its of 2 times and above 2 times blood donors (27.27%) (P<0.05), the TPPA positive rate of the different education background persons had significant difference (P<0.05), the TPPA positive rate of the different ABO blood group persons had no significant difference (P>0.05). Conclusion The anti-TP antibody ELISA screening test exists a certain proportion of false positive. In order to avoid the leakage of weak positive specimens, the blood screening laboratory should monitor the performance of the test, set the scientific and reasonable grey area, and reduce the unnecessary blood waste. In order to inform of the blood donors and reduce loss of the blood donors, and establish a stable and sustained team of the blood donors,it was recommended that syphilis confirmation testing of the blood donors on the unqualified results of anti-TP antibody screening was added in the blood station. The results information feedback of the blood donors should refer to the syphilis confirmation results. The syphilis confirmation result should be referenced as the basis for shielding the unqualified blood donors. The syphilis infection rate is higher among the blood donors on the unqualified results of anti-TP antibody screening, especially married persons and low education background persons. When the test results feedback of the blood donors of the TPPA positive and TRUST titer positive, it was recommended that they were tested in clinical medical institutions as soon as possible, so that reinfection syphilis, latent syphilis (recessive syphilis) persons could be treated in time and avoided disease transmission.

Clinical Significance of Absolute Eosinophil Counts in Patients with Chronic Renal Diseases

Objective To investigate the association between eosinophil counts (EOS), renal function in patients with chronic kidney disease (CKD). Methods A total of199blood samples of patients with CKD in nephrology were collected from October2017to September2018to measure the absolute blood eosinophil counts, renal function by detection of urea, creatinine, uric acid, cystatin C, retinol binding protein. Results Single factor correlation analysis showed that EOS was negatively correlated with estimated glomerular filtration rate(r=-0.206, P=0.003)and positively correlated with urea(r=0.234, P=0.001), creatinine(r=0.233, P=0.001), cystatin C(r=0.200, P=0.007), retinol binding protein(r=0.164, P=0.027). Multilevel linear regression analysis showed that EOS was only negatively correlated with eGFR (P=0.028). Patients with advanced stages of CKD had increased eosinophil counts. Sixteen cases (8.04%) had eosinophil counts more than500/μL, named as peripheral blood eosinophilia (PBE), all of whom presented severe renal dysfunction(CKD stage4or5). Conclusion Elevated eosinophil count is associated with renal functional insufficiency in patients with CKD, routine eosinophil count detection may help evaluate the renal functional insufficiency in patients with advanced CKD or dialysis.

Correlation Analysis of Serum Galectin-3, IL-38 Levels with Pulmonary Function in Patients with Chronic Obstructive Pulmonary Disease

Objective To investigate the changes of serum galectin-3 and IL-38 levels in patients with chronic obstructive pulmonary disease (COPD) and their correlation with pulmonary function. Methods From January 2016 to April 2018,151 patients with acute exacerbation of COPD were selected as acute group, 107 stable COPD patients and 129 healthy people in the same period were selected as stable group and control group. Serum Galectin-3,IL-38,CRP and PCT levels and lung function were measured at the time of enrollment. Pearson correlation was used to analyze the correlation among serum Galectin-3,IL-38,CRP and PCT levels and lung function. Results The serum levels of Galectin-3,IL-38,CRP and PCT were higher in acute group and stable group than in control group (P<0.05). The serum levels of Galectin-3,IL-38,CRP and PCT in acute group were higher than those in the stable group (P<0.05). The levels of PEF% pred, FEV1% pred and FEV1/FVC in acute group and stable group were lower than those of in control group (P<0.05),and the levels of PEF% pred, FEV1% pred and FEV1/FVC in acute group were lower than those in stable group(P<0.05). Serum Galectin-3,IL-38,CRP and PCT levels were negatively correlated with pulmonary function indices (PEF% perd, FEV1% pred,FEV1/FVC,P<0.05). ROC curve analysis showed that the area under curve (AUC) of serum Galectin-3 and IL-38 in the diagnosis of acute exacerbation of COPD was 0.839 and 0.893, the sensitivity was 0.808 and 0.781, and the specificity was respectively 0.841 and 0.748. The AUC,sensitivity and specificity of serum Galectin-3 and IL-38 in the diagnosis of acute exacerbation of COPD were higher than those of CRP (AUC: 0.735, sensitivity: 0.722, specificity:0.710) and PCT (AUC:0.760, sensitivity: 0.702, specificity:0.682). Conclusion Serum Galectin-3 and IL-38 levels increased significantly in COPD patients, which were closely related to lung function. And serum Galectin-3 and IL-38 could be used as a biological marker for estimating the severity of COPD. However, the diagnostic value of serum Galectin-3 and IL-38 in acute exacerbation of COPD are better than that of CRP and PCT.

Value of STAT3 in Diagnosis and Prognosis of Pancreatic Cancer

Objective To analyze the value of signal transducer and activator of transcription 33 in diagnosis and prognosis of pancreatic cancer. Methods From February 2014 to August 2016, specimens from 74 cases of pancreatic cancer, 80 cases of benign pancreatic tumor and pancreatitis (40 cases each) were collected. The expression level of STAT3 in pancreatic cancer and benign disease was detected by immunohistochemical staining, then compared to evaluate the diagnostic efficacy of STAT3 expression. The staging and lymph node metastasis of pancreatic cancer in STAT3 positive and negative subjects were analyzed. STAT3 was compared between 32 patients with poor prognosis (spread, metastasis, recurrence) and 42 patients with good prognosis. Results The expression of STAT3 in pancreatic cancer was higher than that in adjacent tissues, and the difference was statistically significant (P<0.05). There was significant difference in the expression of p-STAT3 in pancreatic carcinoma tissues, paracancerous tissues and benign pancreatic tissues (P<0. 05).The sensitivity, specificity, positive predictive value, negative predictive value and coincidence rate of STAT3 in the diagnosis of pancreatic cancer were 70.3% (52/74), 86.2% (69/80), 82.5% (52/63), 75.8% (69/91) and 78.6% (121/154), respectively. The lymph node metastasis of STAT3 positive pancreatic cancer was higher than that of STAT3 negative pancreatic cancer. STAT3 positive lymph node metastasis in pancreatic carcinoma was higher than that in STAT3 negative group. The clinical stages of STAT3 positive patients in pancreatic carcinoma were statistically different from STAT3 negative ones (P<0. 05). The positive rate of STAT3 in patients with poor prognosis was significantly higher than that in patients with good prognosis (P<0. 05). Conclusion STAT3 is valuable in the diagnosis of pancreatic cancer and the prediction of growth and metastasis.

The Correlation between The Prevalence of Hyperuricemia and The Polymorphisms of SLC2A9 Gene Rs2241480 in Coastal Population

Objective To investigate the prevalence of Hyperuricemia (HUA) in coastal areas of shenzhen, and to explore the correlation between HUA and SLC2A9 gene rs2241480 site polymorphisms, providing scientific basis for HUA diagnosis and treatment. Methods 2 753 people living in coastal areas from march to July 2018 were randomly selected, serum uric acid (UA) level was detected by AU5800 automatic biochemical analyzer, and SLC2A9 gene rs2241480 polymorphisms of HUA patients and 120 non-HUA people were detected by Sequenom Mass Array iPlex Gold technology, and the test results were statistically analyzed. Results among 2 753 people, 425 cases of HUA were detected, the prevalence rate was 15.44%, among which 18.27% is in male and 11.55% was in female, and the difference was statistically significant (χ2=3.782 1, P<0.05). > 60 years old group is the highest incidence of HUA, at 30.67%, followed by 51 to 60 years old group, then 41 to 50 years old, 21.18% and 15.09%, respectively, between each age group are statistically significant differences (χ2=2.168 3 ~3.418 6, P<0.05), and HUA prevalence rates were higher in men than in female at below 60 years old groups , difference was statistically significant (χ2=2.761 5~3.905 2, P<0.05), but > 60 years old group HUA prevalence of women more than men,difference was statistically significant (χ2=2.036 5, P<0.05). The detection rates of CC, CT and TT genotypes at the rs2241480 loci of SLC2A9 in the HUA group were 44.71%, 30.82% and 24.47%, respectively. The detection rates of CC genotypes were 18.33%, 37.50%and 44.17% in the non-HUA group, respectively. The detection rates of CC genotypes in the HUA group were higher than those in the non-HUA group, with statistically significant differences (χ2=5.162 5, P<0.05). The C allele frequency in the HUA group was 60.12%, significantly higher than it is 37.08% in the non-HUA group, and the difference was statistically significant(χ2=3.602 4, P<0.05). Conclusion the prevalence rate of HUA in coastal areas of shenzhen is above the average level in China, and it is younger. SLC2A9 gene rs2241480 locus polymorphism has some correlation with HUA, and CC genotype and C allele might be pathogenic genes of HUA in this region.

Objective To investigate the association of serum level of macro-elements (calcium and magnesium) and trace elements (copper,iron,manganese,selenium,and zinc) with depression of patients. Methods Two hundred and forty seven patients were enrolled and 248 healthy volunteers,matched by age and sex,were taken as control. The serum levels of macro-minerals and trace elements were detected by atomic absorption spectroscopy. Statistical analysis was performed with independent sample t-tests and Pearson's correlation test. Results Significantly decreased concentrations of calcium and magnesium,iron,manganese,selenium,and zinc were found (P<0.05),while copper notably increased (P<0.05) in the depressive patients compared with the control. Positive correlations of Mn with Mg (r=0.168,P=0.011) and Mg with Se (r=0.157,P=0.014) were seen in patients while negative correlations of Mn with Zn (r=-0.187,P=0.006),Mn with Cu (r=-0.164,P=0.008),and Cu with Se (r=-0.152,P=0.007) were noted. The persons in the control group showed significant positive correlations of Mn with Mg (r=0.281,P<0.001) but negative correlations of Mn with Cu (r=-0.284,P<0.001) and Cu with Mg (r=-0.624,P<0.001). Conclusion Our study suggests that altered serum concentrations of macro-minerals and trace elements are major factors that may contribute to the pathogenesis of depression and may help for assessment of depression risk.

Expression of Th1, Th17, Th22 and Treg Cells in Peripheral Blood of Children with ITP and Its Clinical Significance

Objective To explore thefrequency of Th1, Th17 and Treg cells in peripheral blood of children with ITP and its clinical significance. Methods Forty-six children with ITP diagnosed and treated in our hospital from June 2017 to September 2018 were enrolled as a case group, and 40 children in the same period were examined as health controls. The frequency of Th1, Th17, Th2 and Treg cells in peripheral blood mononuclear cells were detected by flow cytometry. The cytokines IFN-γ, IL-4 and IL-17 were detected by enzyme-linked immunosorbent assay (ELISA). The relationship between Th17, Th2, Treg cell ratio, cytokine-level and different periods of disease and platelet levels were analysed. Results The ratio of Th1 cells and Treg cells and the level of IFN-γ、IL-17 in peripheral blood of the case group were lower than that of the control group, and the proportion of Th2, Th17 and the level of IL-4 were higher than that of the control group, the difference was statistically significant (P<0.05). The levels of Th1, Treg and platelet count in peripheral blood of children in case group after treatment were higher than those before treatment, while the levels of Th2, Th17 and Th2 were lower than those before treatment, the difference was statistically significant (P<0.05). The change of platelet count before and after treatment were positively correlated with Treg and Th1 levels (r=0.461, 0.383), and negatively correlated with Th17 and Th2 levels (r= -0.362, -0.311). Conclusion The unbalanced proportion of Th1, Th2, Th17, Treg cells and cytokines in peripheral blood is related to the pathogenesis of ITP in children. T lymphocytes can be used to predict disease status and platelet level.

Clinical Observation of Low Molecular Weight Heparin in the Treatment of AECOPD Combined with Respiratory Failure, Coagulation Index, CK-MB and Inflammatory Factors Expression

Objective To evaluate the low molecular weight heparin in the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated with respiratory failure clinical curative effect and influence on blood coagulation function, pulmonary function, myocardial injury index and inflammatory factor expression. Methods 86 cases of AECOPD patients in our hospital from January 2015 to January 2017 were enrolled as the study group, the patients were randomly divided into control group and observation group, the control group received routine treatment, the observation group in the control group treatment at the same time give low molecular weight heparin; after 7d treatment to evaluate and compare the clinical efficacy of two groups, their lung function index, blood coagulation index, inflammatory factors and myocardial damage index were recorded and analysed. Results The total effective rate of the observation group was 97.67%, and that of the control group was 86.05%. The clinical efficacy of the observation group was better than that of the control group, P<0.05. After treatment, the two groups of patients were better improved in pulmonary function, coagulation function indicators, inflammatory factors and myocardial damage indicators compared to before treatment, P<0.05; but after treatment, the observation group patients were better improved than the control group, P<0.05. Conclusion Low molecular weight heparin can improve the clinical efficacy of AECOPD and respiratory failure patients, reduce the expression level of inflammatory factors, improve the pulmonary function and excessive coagulation state, and reduce myocardial injury.

Practice Discussion of External Red Light Irradiation on Improving the Antioxidant Capacity and Blood Phosphorus Metabolism of Maintenance Hemodialysis Patients

Objective To investigate the influences of external red light irradiationon on antioxidant capacity and blood phosphorus metabolism in maintenance hemodialysis (MHD) patients. Methods 86 MHD patients were selected from October 2015 to April 2018 in our hospital, who were randomly divided into test l group and control group, 43 cases in each group. The control group was given simple routine hemodialysis treatment, while the test group was exposed to external red light irradiation therapeutic apparatus during the hemodialysis. The general physiological conditions (body temperature, heart rate, blood pressure, respiration) and the contents of red blood cells and hemoglobin were observed before and after irradiation of the test group, and the changes of serum oxidative stress indexes [Superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSHPx),advanced oxidation protein product (AOPP)] and serum phosphorus levels before the first dialysis and after 10 times of dialysis were compared between the 2 groups, and the adverse reactions during the irradiation of the test group were observed. Results There were no significant differences in body temperature, heart rate, mean arterial pressure, respiration, red blood cell count and hemoglobin content of the test group between before irradiation and after treatment (P>0.05). The levels of serum SOD and GSH-Px of the control group after 10 times of hemodialysis were significantly lower than those before the first hemodialysis (P<0.05), and the level of serum MDA of the control group after 10 times of hemodialysis was significantly higher than that before the first hemodialysis (P<0.05), and there was no significant difference in serum AOPP level of the control group between after 10 times of hemodialysis and before the first hemodialysis (P>0.05). The levels of serum SOD and GSH-Px of the test group after 10 times of hemodialysis were significantly higher than those before the first hemodialysis and of the control group (P<0.05), and the levels of serum MDA and AOPP of the test group after 10 times of hemodialysis were significantly lower than those before the first hemodialysis and of the control group (P<0.05). The levels of serum phosphorus in both groups were significantly lower after 10 times of hemodialysis than those before the first hemodialysis (P<0.05), of which the test group after 10 times of hemodialysis was significantly lower than that of the control group (P<0.05). No adverse reaction occurred in test group during the red light irradiation process. Conclusion External red light irradiation can significantly enhances the antioxidant capacity of MHD patients, reduces blood phosphorus level and does not produce adverse reactions, with high safety.

Objective To investigate the relationship between the total serum bile acid (TSBA) and its value in pregnant women with intrahepatic cholestasis of pregnancy,and to analyze the influence of bile acid in cord blood on the outcomes of perinatal infants. Methods Clinical data of 123 cases were collected from Dec 2015 to May 2017 and the TSBA and transaminase were detected. The frequencies of perinatal complications were analyzed. The pregnant women were divided into two groups based on the level of TSBA and their clinical menifestations. Group 1 included the patients with 10-40 mol/L TSBA and skin itching whereas group 2,with 40 mol/L TSBA and severe pruritus associated with recurrent ICP and multiple pregnancy risk factors. Ninety six cases of normal pregnant women were taken as control. Results The pregnant women of both groups showed a high TSBA level compared with the control (P<0.05). A positive correlation of bile acid level was found between the neonatal cord blood and maternal peripheral blood (r=0.903,P<0.05). Patients of group 2 had high frequencies of premature birth,neonatal asphyxia,amniotic fluid contamination. Conclusion Neonatal perinatal complications in severe ICP pregnant women were frquently noted and grading of intrahepatic cholestasis during pregnancy may help determine the perinatal prognosis.

Objective To detect the expression pattern of HMMR-AS1 and its effect on the sensibility of chemotherapy on cervical cancer. Methods qRT-PCR was used to detect the expression of HMMR-AS1 in cervial cancer tissues and para-carcinoma normal tissues and EMT markers in Hela cell lines,the relationship between the clinic characteristics of the cervial cancer patients and the expression level and the relationship between the overall survival time and the expression level were analyzed. The effect of high expression of HMMR-AS1 on the proliferation rate of cervical cancer cells was determined by CCK-8 assay. The effect of high expression of HMMR-AS1 on the sensitivity of cervical cancer cells to cisplatin was determined by chemotherapeutic drug sensitivity test. Results HMMR-AS1 was upregulated in cervical cancer (P=0.03),high expression of HMMR-AS1 was correlated with the stage (P=0.031),T (P=0.004),N (P=0.047),M (P=0.033),chemotherapy (P=0.007) and shorter overall survival time (χ2=7.31,P=0.006 9). Upregulation of HMMR-AS1 promoted Hela cell proliferation (96 h,P=0.002),migration(P=0.000 65),invasion(P=0.005 3),and reduced sensitivity to cisplatin (P=0.003 2). Conclusion HMMR-AS1 was upregulated in cervical cancer,and HMMR-AS1 could shorten the overall survival time of cervical cancer patients,and lowered the sensibility of chemotherapy on cervical cancer.

Surveillance of Antibiotic Resistance of Five Clinically Significant Isolates in A Third-class Hospital

Objective To investigate the clinical distribution and drug resistance of the five clinically important bacteria isolates in the hospitals from 2012 to 2016 in order to provide reference for the proper use of antibiotics. Methods Tests of bacterial identification and antibiotic susceptibility were performed with the MicroScan WalkAway 96 PLUS bacterial identification system. All of the data were analyzed by WHONET5.6 software. Results An average of 1 520 isolates were harvested each year. Among them Escherichia coli and Klebsiella pneumonia producing extended-spectrum β-lactamase (ESBL) strains showed a downward trend. The resistance rate of E. coli to carbapenem-based antibiotics was basically stable during the five years studied, and that of K. pneumoniae to carbapenems showed an upward trend. Pseudomonas aeruginosa presented decreased resistance to all of the monitored antimicrobials, including imipenem and meropenem. Carbapenem resistance rate of Acinetobacter baumannii gradually increased from 32.8% to 82.3%. The positive rate of methicillin-resistant Staphylococcus aureus dropped from 45.2% in 2012 to 37.5% in 2016. Conclusion The prevalence of carbapenem-resistant K. pneumoniae and A. baumannii is increasing and of great concern in recent years.

Objective To explore the application of sigma performance verification diagram (SPVD) to evaluation of Roche and Beckman automatic biochemical analyzers. Methods The data of indoor and external quality control of ten items were collected, including AFP, tPSA and fPSA, CEA, CA199, CA125, CA153, iron, protein, total beta HCG, and free estriol. The SPVD of the above markers was prepared based on the detections of Roche and Beckman analyzers, using the CV estimated in the laboratory, percentage difference as bias estimates, and TEa provided by NCCL and the Health Industry Standards. Results Two of the 4 external quality evaluation items provided by Roche analyzer detections were found to have poor results and one was qualified before improvement. After improvement, however, all of the 4 items showed excellent results. For Beckman biochemical analyzer, eight items of external quality assessment were close to the critical value and the quality of 3 items was not acceptable before improvement. The quality of eight items was still in the vicinity of critical point although it seemed to be improved after improvement. Conclusion The detection performance of Roche e602 analyzer appears to be superior to that of Beckman DXI 800, and sigma performance verification diagram may distinctively demonstrate the quality of detections.

Objective To study the count of peripheral eosinophils in patients with asthma underlying chronic obstructive pulmonary diseases (COPD) and explore its significance in prognosis of COPD. Methods Eight seven patients with COPD hospitalized in 2016 and 2018 were divided into group A(EOS≥2%)and group B (EOS<2%). The relationship was compared between the count of peripheral eosinophils and the prognosis of asthma combined with COPD. Results No difference of gender,age,smoking,and duration was noted between the two groups(P>0.05). The EOS counts in group B were significantly less than those in group A but NLR,D-dimer and hs-CRP levels were notably higher than group A(P<0.05). The numbers of inpatients and deaths within one year were found to be increased in group A (P<0.05). Conclusion The number of eosinophils is closely related to the frequency of re-admission,D-dimer,NLR,and hs-CRP,suggesting its potential clinical significance.

Clinical Significance of HBsAg and HBV DNA Quantitation in Patients with Chronic Hepatitis B and HBV-Related Liver Cirrhosis

Objective To investigate the clinical significance of quantitative detection of HBsAg and HBV DNA in the patients with chronic hepatitis B and liver cirrhosis. Methods The levels of serum HBsAg and HBV DNA were retrospectively analyzed in 74 patients with chronic active hepatitis B without liver cirrhosis (group A),39 cases of cirrhosis at compensated stage (group B),and 39 cases at decompensated stage (group C). Results The loads of HBsAg and HBV DNA gradually decreased with the disease exacerbation(P<0.05); the quantitative levels of HBsAg and HBV DNA were closely correlated in patients of the three group(r=0.614,P<0.001). The level of HBsAg,however,did not show any correlation with that of HBV DNA in the patients of groups B(r=0.267,P=0.101) and C(r=0.232,P=0.155),in which the levels of HBsAg and HBV DNA exhibited a negative correlation with the severity of liver cirrhosis. Conclusion sThe level of HBsAg gradually decreases with the severity of liver cirrhosis in the patients with chronic hepatitis B,compensated cirrhosis,and decompensated cirrhosis. The load of HBsAg is positively correlated with that of HBV DNA.

A Predictive Value of Blood Glycemic Variability on The Recent Major Cardiovascular Adverse Events in Acute Coronary Syndrom Patients

Objective To explore the predictive value of glycemic variability on the recent major cardiovascular adverse events in acute coronary syndrom patients. Methods From July 2016 to March 2018, a total of two hundred and forty-seven ACS patients in hospital were included. According to the 30-day follow-up whether occuring MACE, patients were classsfied into MACE group (n=28) and non-MACE group (n=219), the general clinical data, biochemistry and ultrasonic and glycemic variability indexes were compared between two groups, the independent risk factors of MACE during 30-day follow-up of ACS patients were analyzed by using Logistic regression analysis. The predictive value of glycemic variability on MACE was assessed by using ROC curve. Meanwhile, the survival analysis were implemented by using Kaplan-Meier. Results The triple-vessel disease, GRACE scores on admission in the MACE group were significantly higher than in the non-MACE group (P<0.05). Compared to the non-MACE group, the levels of LVEDD, fasting blood glucose, BNP, CRP, blood glucose standard deviation (BGSD), mean amplitude of glycemic excursion (MAGE) were increased and LVEF were decreased in the MACE group, the differences between the two groups were statistically significant (all P<0.05). Logistic regression analysis showed that triple-vessel disease, GRACE scores, BNP, CRP, BGSD, MAGE were independent risk factors for MACE in ACS patients during 30-day follow-up (all P<0.05). The ROC curve analysis showed that BGSD predicted MACE with AUC was 0.883 (95CI: 0.836~0.920, P<0.000 1), the cut-off value were 2.07 mmol/L, which yield sensitivity and specificity were 82.1% and 79.5%, respectively. The cumulative risk of MACE during 30-day follow-up increased markedly who over the cut-off value of glycemic variability (P<0.05). Conclusion There is closely correlation between glycemic variability and recent major cardiovascular adverse events in acute coronary syndrom patients, its can be used as a factor for the poor prognosis of patients.

Objective To study the efficacy of bortezomib-combined chemotherapy in treatment of multiple myeloma and its effect on the levels of VEGF, IL-6 and IL-17. Methods Eighty inpatients were divided into routine group and bortezomib group. The drugs in the bortezomib group included thalidomide, bortezomib, doxorubicin and dexamethasone. VEGF, IL-6 and IL-17 were detected by ELISA and M protein was tested by agarose gel electrophoresis. Determination of globulin was performed by bromocresol green method, the number of bone marrow plasma cells (BMPCs) was examined by bone marrow puncture, and hemoglobin was detected by immunoturbidimetric assay. Results The levels of VEGF, IL-6 and IL-17 in the bortezomib group were significantly decreased compared with those in the routine chemotherapy group (P<0.05) after treatments. The levels of M protein, BMPCs and globulin were notably lower but the hemoglobin was higher in the bortezomib group than in the routine group (P<0.05). The total therapeutic efficacy of bortezomib group was distinctively elevated. Additionally, bortezomib-combined therapy remarkably reduced the frequency of adverse reactions (P<0.05). The levels of VEGF, IL-6 and IL-17 were found to be positively related to the severity of the disease. Conclusion Bortezomib-combined chemotherapy is effective and safe in treatment of multiple myeloma.

Objective To compare the consistency of the results of hypersensitive c-reactive protein (hs-CRP) detected by three different instruments. Methods From march to July, 2018, 156 cases of patients with infection and fever who visited the outpatient department of Longhua district people's hospital in Shenzhen were selected. Using AU5800 as the reference instrument and VITROS5600 and 5390 as the comparison instruments, the consistency of hs-CRP test results of three different instruments was analyzed. Results In serum of 156 cases of hs-CRP reference instrument test results concentration was 93.64±79.74 mg/L, Mindray 5390 automatic blood corpuscle-hypersensitive c-reactive protein all-in-one test results was 87.98±76.57 mg/L, there was no statistically significant difference between the two (t=0.912 4, P>0.05), while VITROS5600 dry piece of biochemical analyzer test results was 182.38±158.79 mg/L, significantly higher than the reference instrument, the difference was statistically significant (t=6.805 6, P<0.05). The correlation coefficient (r) between hs-CRP concentration detected by the three instruments was 0.968 5. The correlation coefficient (r) between VITROS5600 and AU5800 was 0.953 0. Conclusion The results of hs-CRP detected by 5390 hemocyte-hypersensitive c-reactive protein integrator and AU5800 biochemical analyzer were consistent, while the results detected by VITROS5600 dry platebiochemical immune analyzer were obviously higher, but the hs-CRP detected by 3 different instruments showed good positive correlation.
