The Value of Combined Determination of Homocysteine,Lipoprotein (a) and Lipoprotein Phospholipase A2 in the Diagnosis of Coronary Heart Disease

Objective To investigate the diagnostic value of serum homocysteine (Hcy),lipoprotein a (LPa) and lipoprotein phospholipase A2 (LP-PLA2) detection in coronary heart disease. Methods A total of 214 CHD patients admitted to our hospital from March 2016 to March March 2018 were selected as observation group. According to the disease type,82 patients with acute myocardial infarction were treated as group A and 74 patients with stable angina were treated as group B. 58 Cases with unstable angina were treated as group C,and another 180 patients without coronary heart disease in our hospital during the same period were selected as control group. Serum levels of Hcy,LPa,and LP-PLA2 were measured in each group,and comparative analysis was performed. The positive rates of single-marker detection and combined detection were compared. The range of serum Hcy,LPa,and LP-PLA2 indicators was also compared in each group.Results The positive rates of serum Hcy,LPa,and LP-PLA2 detection and combined detection in group A,B,and C were higher than those in the control group,and the difference was statistically significant (P<0.05). The levels of serum Hcy,LPa,and LP-PLA2 were positive and positive in the groups A and C. The rate was higher in group B than in group B (P<0.05). The positive rates of serum Hcy,LPa,and LP-PLA2 in group A,B,and C were higher than those in serum Hcy,LPa,and LP-PLA2 alone. Statistical significance (P<0.05); Serum Hcy level in group A was higher than that in group B and control group,the difference was statistically significant (P<0.05); LPA,LP-PLA2 levels in group A,B,and C were higher than those in control group The difference was statistically significant (P<0.05); there was a clear line between the LPA levels in the A,B,and C groups,and there was a clear line between the LP-PLA2 levels in the B and C groups.Conclusion The combined detection of serum Hcy,LPa,and LP-PLA2 levels has significant value in the diagnosis of coronary heart disease. LPa and LP-PLA2 levels can effectively distinguish the type of coronary heart disease,and it is worth promoting.

Analysis of Association of Serum Levels of DKK1 and β2-microglobulin with the Progress of Multiple Myeloma Disease

Objective To investigate the correlation between serum levels of DKK1 and β2-microglobulin and the multiple myeloma disease progress. Methods Sixty-six patients with multiple myeloma were selected as study group,and sixty-six healthy subjects were selected as control group. The serum levels of Wnt pathway inhibitor Dickkopf-1 (DKK1) and beta2-microglobulin were measured,and their associations with the incidence of multiple myeloma were analyzed. Results ① The serum levels of DKK1 and β2-microglobulin in the study group were significantly higher than those in the control group (P<0.05). ② The differences of serum DKK1 and β2-microglobulin in different myeloma patients were statistically significant (P<0.05); After multiple comparison,the serum levels of DKK1 and β2-microglobulin in stage II and III were all higher than those in stage I (P<0.05),and the levels of DKK1 and β2-microglobulin in stage III were all higher than those in stage II (P<0.05). ③The difference of serum DKK1 and β2-microglobulin in different stages was statistically significant (P <0.05); After multiple comparison,the serum levels of DKK1 and β2-microglobulin in patients in renal insufficiency stage were higher than those in other periods (P<0.05). Serum DKK1 and β2-microglobulin levels in patients in early stage and recurrence stage were higher than those in clinical remission stage (P<0.05). Conclusion Serum DKK1 and β2-microglobulin levels are positively correlated to the severity of multiple myeloma. The detection of the two markscan effectively evaluate the patient's condition

The Efficacy of Insulin Glargine Combined with Glimepiride on Glycemic Control and Levels of SPARC Protein in Elderly Patients with Type 2 Diabetes

Objective To investigate the effect of insulin glargine combined with glimepiride on glycemic control and serum levels of cysteine-rich acidic protein (SPARC) in elderly patients with type 2 diabetes. Methods One hundred and twenty six patients with type 2 diabetes were randomly divided into control group and observation group on average. The patients in the control were given glargine treatment while those in observation group received both insulin glargine and glimepiride. Fasting blood glucose (FBG),2 hour postprandial blood glucose (2 hPG),HbA1c,and SPARC levels were compared between the two groups before and after 3 months of treatments. The effects and adverse reactions of the two groups of patients were compared. Results The effective rate was 79.4% in the control and 90.5% in the observation group after treatments (P<0.05). No significant difference of FBG,2 hPG,HBA1C and SPARC levels was seen between the two groups before treatment (P>0.05). After treatment,however,the levels of FBG,2 hPG,HBA1C,and SPARC in the two groups were significantly decreased (P<0.05). No hypoglycemia occurred in all of the patients. Conclusion The therapeutic effect of insulin glargine combined with glimepiride is promising in glycemic control of elderly patients with type 2 diabetes,which may reduce insulin resistance in diabetic patients with high safety.

Effects of Batroxobin and Vasodilator on Sudden Deafness and Coagulations

Objective To investigate the efficacy of batroxobin and vasodilators in the treatment of sudden deafness and coagulations. Methods Eighty-four patients with sudden deafness admitted to the hospital from 2015 to 2017 were randomLy divided into two groups,with 42 cases in each. The control was treated with vasodilators and the observation group was with batroxobin. Clinical efficacy and changes in coagulation parameters of pre- and post-treatments were compared between the two groups. Results The clinical curative effect of the observation group was higher than that of the control (P<0.05). The prothrombin time,thrombin time and activated partial thromboplastin time in the observation group were prolonged than those in the control. The levels of fibrinogen and platelet factorswere found to be lowedin the observation group (P<0.05). Conclusion The efficacy of batroxobin treatment in patients with sudden deafness is superior to that of vasodilators,which can effectively improve the coagulation function of patients and is worthy of clinical application.

Analysis of Chorionic Chromosomes and Related Immune Indicators in Patients with IVF Embryo Abortion

Objective To investigate the relationships between missed abortion and age,autoimmune antibody,the history of spontaneous abortion among women after invitro fertilization-embryo transfer(IVF-ET). Methods A retrospective analysis was made on 113 cases of chorionic chromosome examination in early abortion after IVF-ET from February 2016 to August 2018. Anti-nuclear antibody(ANA),anti-cardiolipin antibody(ACA),anti-thyroid peroxidase antibody(ATA) and anti-thyroglobulin antibody(ATG) were also detected. Results ①64 patients had normal chromosomes in 113 cases. Chromosome abnormalities were found in 49 cases. ②Chromosome abnormality rate of patients over 35 years old was significantly higher than that of patients under 35 years old(67.57% VS 31.58%,P<0.000 1); ③There was no significant correlation between the history or times of early abortion and the chromosomal abnormalities. ④The positive rates of ACA,ANA and ATA/ATG in normal chromosome groups were higher than those in chromosomal abnormality groups,but without significant difference. Conclusion The rate of chromosome abnormalities increases with age. There is no significant relationship between the history of early abortion and the abnormalities of chorionic chromosomes after IVF-ET.

The Relationship of Levels of LP(a),Lp-PLA2 and HFABP with the Severity of Coronary Heart Disease

Objective To analyze the relationship between the levels of LP (a),Lp-PLA2 and HFABP and the severity of coronary heart disease (CHD). Methods One hundred CHD patients diagnosed by coronary angiography in the hospital in recent three years were selected for observation and 100 healthy volunteers as the control. Sera of fasting venous blood were collected after admission,and the level of HFABP were measured by ELISA. The Lp-PLA2 and LP(a) were detected by fluorescent enhanced immunochemiluminescence and immunoturbidimetric method,respectively. Result The levels of LP (a),Lp-PLA2 and HFABP in the observation group were found to be significantly higher than those in the control (P<0.05); the increase of LP (a),Lp-PLA2 and HFABP was positively correlated with the severity of CHD (P<0.05). Conclusion The levels of LP (a),Lp-PLA2 and HFABP in CHD patients were notably elevated which is closely associated with the severity of CHD,suggesting a potential significance in the diagnosis of CHD.

The Clinical Diagnostic Value of Serum Anti-C1q Antibody Level in SLE and LN Patients

Objective To detect the associations between anti-C1q antibody and systemic lupus erythematosus(SLE)and that between anti-C1q antibody and lupus nephritis(LN).Methods The Anti-C1q antibody were measured using ELISA kit from 110 cases of SLE patients(SLE group) and 60 cases of healthy people(normal control group). The 110 SLE patients were divided into LN group with 41 patients and non LN group with 69 patients. The serum levels of Anti-C1q,anti-dsDNA antibody and biochemical markers of the two groups were detected,and the result were analyzed. Results The serum anti-C1q antibody level of SLE group was significantly higher than that of normal control group[(11.90±23.83)U/mL vs(2.73±4.18)U/mL,P<0.05].Serum level of anti-C1q antibody in LN group showed a strongly positive correlation with anti-dsDNA antibody and showed a negative correlation with C3 and C4 level.Conclusion Anti-C1q antibody would be helpful to characterize the disease activity of SLE,and is associated with SLE kindey damage. The combined detection of serum anti-C1q antibody,AnuA and anti-dsDNA antibody can improve LN diagnostic specificity.

An Investigation of 25-Hydroxyvitamin D Level in Shandong Province

Objective To explore the association of serum 25- (OH) vitamin D level with age,gender and seasons in 5 181 persons of Shandong province. Methods Serum samples were collected from the hospital from 2014 to 2017,and the serum 25- (OH) vitamin D was detected by electrochemiluminescence. All of the subjects were grouped based on age,gender,and the seasons of sampling for analyses of vitamin D. Results Males had a significantly higher level(17.11±11.75 ng/mL) of vitamin D than females (15.08±11.73 ng/mL) (P<0.01). The vitamin D level in Children of age of 0~9 (26.99±17.28 ng/mL) was strikingly elevated (P<0.01) but no significant difference was noted among the other groups(P>0.05). The vitamin D level in all subjects varied in seasons,showing the highest in the summer and the lowest in the winter (P<0.01). Conclusion The vitamin D level varies in genders,ages and seasons,suggesting a deficiency of vitamin D nutrient in the local people of Shangdong.

Clinical Application of Tuberculosis - Interferon Gamma Release Assay in The Diagnosis of Tuberculosis

Objective To evaluate the clinical value of tuberculosis-interferon gamma release assay (TB-IGRA) in the diagnosis of tuberculosis. Methods A retrospective analysis of the clinical data from 756 hospitalized patients,who received TB-IGRA test from Sep 2015 to Jun 2017,was performed. Results The positive rate of TB-IGRA was 87.31% in TB patients,and the positive rate of TB-IGRA was 85.55% and 90.80% respectively in the patients with pulmonary tuberculosis and extrapulmonary tuberculosis,significantly higher than that in the patients without tuberculosis (P<0.001); there was significantly higher detectable rate in TB patients by TB-IGRA than by anti-acid staining,detection of TB antibody, and the detection of ADA in hydrothorax and ascites (P<0.001); there were no statistical differences in the detection level of TB-IGRA between different ages,sex,basic diseases,tuberculosis sites,lymphocyte counts,and tuberculosis activity.Conclusion sTB-IGRA had higher diagnostic value in the detection of pulmonary tuberculosis and extrapulmonary tuberculosis. TB-IGRA can be used as auxiliary diagnosis of tuberculosis.

Significance of Plasma CG and TBA in Patients with Intrahepatic Cholestasis of Pregnancy

Objective To investigate the level and significance of plasma cholyglycine (CG) and total bile acid (TBA) in intrahepatic cholestasis of pregnancy (ICP) which may provide a basis for the clinical diagnosis and treatment of ICP.Methods Eighty ICP patients and 80 healthy pregnant women were selected from 2015 to 2017 and were divided into observation group and control group. The plasma levels of CG,TBA and perinatal complications in the two groups were compared. ICP patients were grouped according to the levels of CG and TBA,and the perinatal complications were compared. Results The levels of serum CG and TBA in the observation group were significantly elevated when compared with those in the control (P<0.05). The incidence of fetal distress,premature birth,neonatal asphyxia and neonatal death were significantly higher in the observation group than those in the control (P<0.05). The incidence of fetal distress,premature delivery and neonatal asphyxia increased with the high level of CG (P<0.05). The level of TBA in patients with ICP was found to be in parallel with the incidence of fetal distress,premature delivery and neonatal asphyxia (P<0.05). Conclusion Patients with ICP have a high level of serum CG and TBA compared with normal pregnancies. Detection of CG and TBA is useful for clinical diagnosis and prognosis of ICP.

Investigation of Human Papillomavirus Infection and its Genotypein 6 912 Patients from a Hospital in Hefei,2018

Objective To analyze the infection status of human papillomavirus (HPV) in outpatient department of obstetrics and gynecology of a hospital in Hefei. Methods The cervical exfoliated cells were collected from 6 912 obstetrics and gynecology outpatient patients in 2018,DNA was extracted,HPV genotypes were detected,and infection rates and subtypes were analyzed. Results 2 627 were found to be HPV positive from 6 912 female patients,with a total positive rate of 38.0%. Among them,the high-risk HPV52 infection rate was 8.0%,the total positive rate and high-risk infection rate were 53.02% and 50.48% in the group of 56 years old above,and the total positive rate and high-risk infection rate were 33.09% and 29.96% respectively in the group 26~35 years old. As the number of infection types increased,the infection rate of each group decreased. The rate of single infection was the highest,it was 1 782 cases (25.78%),that is, and the rate of multiple infection was the lowest,it was 236 cases (3.41%), Conclusion There are different e distributions of HPV in outpatients of this hospital in type and age,and the infection is mainly high-risk type and single infection,which provides good theoretical support for the selection of HPV vaccination in this region.

Study on the Clinical Significance of Combined Detection of SCC,CEA and CYFRA211 For Cervical Cancer Screening

Objective To explore the clinical significance of combined detection of SCC,CEA and CYFRA211 for early cervical cancer screening. Methods 108 patients with high risk of cervical cancer screened in our hospital were divided into cervical cancer group(67 cases) and Cervix benign disease group(41 cases),and 82 healthy people who had physical examinationin our hospital at the same period were selected as the control group,and the normal cervix was tested by TCT test. 3-5mL blood were collectedfrom all ofsubjects ,and levels of squamous cell carcinomaantigen (SCC),cytokeratin 19 fragment (CYFRA211) and carcinoembryonic antigen (CEA) were measured by chemiluminescence. The serum SCC,CEA and CYFRA211 single item detection or joint detection of tumor markers to the cervix in the cervix was compared. Results Levels of serum SCC,CYFRA211 and CEA in the cervical cancer group were higher than those of the benign cervical disease group and the control group,and the cervical benign disease group were higher than the control group,the difference were statistically significant (P<0.05). The serum levels of SCC,CYFRA211 and CEA in the low differentiation group were higher than the middle differentiation group and the high differentiation group,and the middle differentiation group was higher than the high differentiation group,and the difference was statistically significant (P<0.05). Serum SCC and CYFRA211 levels in squamous carcinoma group were higher than adenocarcinoma group and squamous adenocarcinoma group,serum CEA level in adenocarcinoma group were higher than squamous carcinoma group and squamous adenocarcinoma group (P<0.05). There was no difference in serum SCC and CYFRA211 between adenocarcinoma group and squamous adenocarcinoma group. There was no difference in serum CEA between squamous carcinoma group and squamous adenocarcinoma group. The sensitivity,specificity,positive predictive value and negative predictive value were higher than those of single detection of serum SCC,CEA or CYFRA211,and the difference was statistically significant (P<0.05) when the three groups were combined to detect the cervical cancer screening standard. Conclusion The combined detection of serum SCC,CEA and CYFRA211 has clinical significance for early cervical cancer screening,and is helpful for differentiation of early cervical cancer and differential diagnosis of pathological types.

Establishment and Preliminary Application of Intestinal Rotavirus (RV) Nested PCR Detection Method

Objective To establish a high specificity and sensitivity of detection of infant diarrhea stool specimens in the gut of rotavirus (RV) nested polymerase chain reaction (PCR),and to explore its preliminary application in clinical diagnosis. Methods according to the RV conservative district gene sequences and USES Primer5.0 software design two pairs of primers of nested PCR to establish a high specificity and sensitivity of detection of RV nested polymerase chain reaction (PCR),and to evaluate its specific degrees and sensitivity. 136 clinical samples were tested with the established nested PCR method,and the test results were verified by single rt-pcr,colloidal gold and gene sequence analysis. Results the RV specific bands of 192bp were detected by nested PCR,and the other enteroviruses were negative and specific. The RV of 8.705fg/ul can be detected at least by nested PCR method. The detection results of nested PCR and single rt-pcr were 100%,but lowerthan the colloidal gold method,and the difference was statistically significant (χ2=2.615,P<0.05). The RV products amplified by the nested PCR method were sequenced by nucleotide. By BLAST comparison,the homology of more than 20 known sequences,such as RV and EU679386,DQ870492 and AF531912,was 98%~100%. Conclusion the detection method of nested PCR for intestinal virus (RV) was established,which was characterized by high specificity,high sensitivity and rapid,and worthy of clinical application.

Effect of Cardiopulmonary Resuscitation Combined with Salvia Miltiorrhiza Ligustrazine Injection on Patients with Acute Craniocerebral Injury and its Effect on GCS Score and Apache Ⅱ Score

Objective To study the clinical effect of cardiopulmonary resuscitation combined with salvia miltiorrhiza ligustrazine injection in the treatment of emergency craniocerebral injury and its effect on GCS score and Apache Ⅱ score.Methods152 patients admitted in our hospital from March 2013 to March 2018 were randomly divided into two groups: observation group (n=76) and control group (n=76).The control group was treated with cardiopulmonary resuscitation and symptomatic support,while the observation group was treated with salvia miltiorrhiza ligustrazine injection on the basis of the control group.Treatment 7 days. The indexes of hemodynamics,lactic acid,GCS and Apache Ⅱ,brain edema area and prognosis were compared between the two groups.Results Before treatment,there was no difference in TK,Fi,MST,lactic acid,blood glucose,APTT,TT,PT level and brain edema area between the two groups (P>0.05). After treatment,the levels of MST,Fi,lactic acid,blood glucose,APTT,TT,PT and brain edema in the observation group were lower than those in the control group(t=8.262,P<0.05;t=7.757,P<0.05;t=10.127,P<0.05;t=3.526,P<0.05;t=9.162,P<0.05;t=8.447,P<0.05;t=9.142,P<0.05;t=29.015,P<0.05).The level of TK is higher in the observation groupthan that in the control group(t=9.698,P<0.05).After 3 months of treatment,the rate of good and moderate disability in the observation group was 76.32%,which was higher than that in the control group (55.26%)(χ2=7.480,P<0.05).Conclusion Cardiopulmonary resuscitation combined with salvia miltiorrhiza ligustrazine injection has a good clinical effect in the treatment of emergency craniocerebral injury.

Objective The Objective of this research is to analyze the IgM antibody test of 9 respiratory pathogens causing child respiratory tract infections,in the selected tertiary hospital to provide guidance for the clinical treatment of these infections. Methods This research the indirect immune-fluorescence technique was used to detect IgM against 9 respiratory tract infection pathogens in the serums of 4 391 outpatient and inpatient child respiratory infection cases,including mycoplasma pneumonia (MP),syncytial virus (RSV),Chlamydia pneumonia (Cpn),Coxiella Burnetii (COX),Influenza B Virus (IFVB),Legionella pneumophila (LP1),adenovirus(ADV),influenza A virus (IFVA),and parainfluenza virus (PIVs). These pathogens were studied for the distributions of their detections from serums taken from different gender,different age groups,and different seasons,and their co-infections. Results 1 564 cases among all the 4,391 cases of child respiratory infections were tested positive,positive rate is 35.62%. The positive rate for males and females were 31.73% and 40.83% respectively. The difference is statistically significant (P<0.05). The top three pathogens causing child respiratory tract infections were MP (1 449 cases,33.00%),PIVs (348 cases,7.93%),IFVB (87 cases,1.98%). The primary co-infection was the co-infection of MP and PIVs,accounting for 56.01% of all infections. The positive rate was highest in the 1-3 year-old age group (P<0.05%) and highest in autumn (P<0.05). Conclusion The primary pathogens causing child respiratory tract infection in the selected tertiary hospital were MP,PIVs,and IFVB. There were statistically significant differences in the detection rate of these pathogens among different age groups and in different seasons. Clinical treatments thus should be customized for child patients with respiratory tract infections basing upon their pathogen test results so as to optimize the use of anti-bacterial drugs,and avoid multiple infections.

Natural Resistance of HBV to LMV/ADV Treatments and Its Association with Genetic Structure in Shenzhen Region

Objectiv To understand current situation of natural resistance of hepatitis B virus (HBV) to lamivudine (LMV) and/or adefovir(ADV)and its association with the virus genotypes. Methods Five hundred and three cases of HBV infection without antiviral treatments were analyzed. The HBV genotypes and viral resistance gene mutations were examined by PCR-reverse dot hybridization gene chips. Results Sixty four of 503 patients (12.72%) with HBV infection were found to carry natural LMV resistance,with the frequency of 73.44%,significantly higher than that of 23.44% in ADV and that of 3.13% in combination of both LMV and ADV (P<0.01~0.05). Genotype B was seen in 68.59% of CHB patients,which was markedly higher than type C (23.06%),type D (2.59%) or type B+C (5.77%)( P<0.05~0.01). Type B dominates in gene mutations of natural resistance to LMV and ADV,among them rtM204V mutation was most frequently noted in 36.17% of LMV natural resistance (P<0.01~0.05) whereas rt236T mutation was detected in 46.67% of ADV natural resistance (P<0.01~0.05). Two patients with LMV and ADV resistance showed type B,carrying rtM204V+rtN236T mutations.Conclusion sPatients with chronic hepatitis B in Shenzhen have a high frequency of natural resistance to LMV and/or ADV. Gene mutation mainly occurs in genotype B,with the sites of rtM204V and rtN236T. Detection of natural drug resistance combined with genotyping before treatment is of clinical significance in enhancing the therapeutic effect of CHB.

An Analysis of Serum Concentrations of APN,CysC,OPN and IgA1 in Children with Henoch Schonlein Purpura Nephritis

Objective To investigate the concentrations of serum adiponectin (APN),cystatin C (CysC),osteopontin (OPN) and immunoglobulin A1 (IgA1) in children with purpura nephritis and explore their significance in the diagnosis of the disease. Methods Thirty-nine children who received treatments in the hospital during the past two years were selected as HSPN group and 50 healthy subjects were enrolled in parallel. Enzyme-linked immunosorbent assay was used to detect the levels of APN,CysC,OPN and IgA1 in the two groups. The incidence of HSPN and the concentrations of APN,CysC,OPN and IgA1 were analyzed by SPSS Pearson correlation analysis software. Results Serum adiponectin in HSPN group was lower than that in control group (P<0.05). The levels of cystatin C,osteopontin and immunoglobulin A1 in HSPN group significantly increased (P<0.05). The incidence of HSPN was negatively correlated with the serum APN (P<0.05),but positively correlated with the concentrations of CysC,OPN,and IgA1 (P<0.05). Conclusion Detection of serum APN,CysC,OPN and IgA1 is useful in assessment of occurrence,progress and severity of the disease.

Values of 21 Genetic Markers Scoring for the Risk of Recurrence in Breast Cancer

Objective To investigate the value of expression of 21 genes for risk evaluation of recurrence in breast cancer patients. Methods Ninety patients with positive estrogen receptor (ER) and negative human epidermal growth factor receptor 2 (HER2) were investigated from June 2014 to August 2017. All patients underwent cancer surgery. The expression of 21 genes (including 16 breast cancer associated genes and 5 references) was detected by real-time fluorescence polymerase chain reaction,and the results were used for assessments of recurrence risk. The patients were divided into low risk (n=26),intermediate risk (n=30) and high risk groups (n=34) according to the scores and risk of recurrence. The recurrence scores of 21 genetic markers were analyzed with multivariate logistic regression analyses. Results The results of single gene test showed that there were no significant differences of recurrence scores in the 21 gene markers among age,pathological types,histological grades,P53,TOPIIa,EGFR,CK5/6 and surgical methods of breast cancer (P>0.05). The recurrence scores of 21 markers were closely related to the tumor sizes,Ki67 and PR positive rates in the patients (P<0.05). Conclusion Detection of the 21 gene markers and recurrence scoring are useful for early diagnoses and treatments of patients with breast cancer.

Objective To investigate the therapeutic effect and correlation analysis of peripheral lymphocyte and neutrophil ratio (LNR) and ratio of lymphocytes to monocytes in patients with nasopharyngeal carcinoma before treatment.Methods We retrospectively analyzed the blood routine results of 160 cases of nasopharyngeal carcinoma treated by radiotherapy and chemotherapy of nasopharyngeal carcinoma in Cancer Hospital of Anhui Province from December 2015 to October 2017. The clinical stage included in this study was stage Ⅰ in 6 cases,21 cases of stage Ⅱ,62 cases of stage Ⅲ and 71 cases of stage Ⅳ. The levels of WBC,NEUT% and LY% of peripheral blood were detected by Sysmex-XE 5000 hematology analyzer,Monocyte percentage (MONO%) and other indicators,calculate the lymphocyte to neutrophil ratio (LNR),lymphocyte to monocyte ratio (LMR). Results There were no significant differences in WBC,NEUTR%,LYR% and LNR between NPC patients and controls (P>0.05). LNR and LMR of patients with stable nasopharyngeal carcinoma (P<0.05). Conclusion sThe determination of peripheral blood lymphocyte to neutrophil ratio,lymphocyte to monocyte ratio before treatment in patients with nasopharyngeal carcinoma can be used as evaluation index Pharyngeal cancer patients with progression-free survival and prognosis of useful indicators,

Objective To study the expression of nitric oxide synthase(NOS)different isoforms in esophageal epithelium and its correlation with the severity of gastroesophageal reflux disease(GERD). Methods 80 cases of GERD patients in our hospital during January 2015 to January 2017 were chosen as observation group,50 cases healthy people underwent physical examination at the same period were chosen as normal control group. The expression of nNOS and iNOS in esophageal epithelium in two groups were measured,serum contents of gastrointestinal function,oxidative stress were measured,expression of proliferation genes in esophageal epithelium were detected.Relationship between nNOS and iNOS nRNA in GERD patients and those disease related indexes were evacuated by Pearson test. Results the observation group of patients with lesions of esophageal epithelial expression of nNOS,iNOS mRNA in esophageal epithelium in observation group were higher than those in normal control group(P<0.05); serum contents of gastrointestinal function indexes such as MTL,GAS,PG I and PG II were significantly lower than those in normal control group,oxidation stress indexes such as SOD was lower than that in normal control group,LPO,MDA were higher than those in normal control group(P<0.05); expression of proliferation genes such as Ki-67,Caspase-3,WISP2 mRNA in esophageal epithelial were higher than those in normal control group. The Pearson test showed that the expression of nNOS and iNOS in esophageal epithelium in GERD patientswere directly related to the severity of the disease. Conclusion There are abnormally high expression of nNOS and iNOS in esophageal epithelium in GERD patients,and the degree of specific expression is directly related to the severity of the disease. It is one of the important causes of the occurrence and development of GERD.

Effect of IVIG Combined Antiviral Regimen on Disease Duration and Laboratory Parameters in Hand Foot Mouth Disease Children with Encephalitis

Objective To investigate the effect of intravenous immunoglobulin for intravenous injection (IVIG) combined with antiviral regimen on the course and laboratory parameters of children with hand-foot-and-mouth disease (HFMD) complicated with encephalitis. Methods A total of 126 children with HFMD and encephalitis who were treated in our hospital from March 2016 to January 2018 were randomly divided into the control group and the observation group,with 63 cases in each group. The control group was given antiviral therapy,and the observation group was given IVIG on the basis of the control group for 3-5 days. The changes of laboratory indexes,course changes and clinical efficacy were observed in the two groups. Results There were no significant differences in age,gender,disease duration,child type,and place of residence between the two groups (P>0.05). Beforetreatment,serum alkaline phosphatase (ALP),prealbumin (PA),white blood cell count (WBC),C-reactive protein (CRP),there was no significant difference between two groups in S100B,Neurospecific Enolase (NSE) and cerebrospinal fluid S100B and NSE (P>0.05). After treatment,ALP and PA increased slightly in both groups. The group was slightly higher than the control group but the difference was not statistically significant (P>0.05). WBC,CRP,serum S100B,serum NSE and cerebrospinal fluid S100B,cerebrospinal fluid NSE decreased,and the observation group was significantly lower than the control group. Academic significance (P<0.05). The time of fever,rash regression time and recovery time of nervous system symptoms in the observation group were significantly shorter than the control group,and the difference was statistically significant (P<0.05). The total effective rate was 77.78% in the control group and 96.82% in the observation group. The difference in clinical efficacy between the two groups was statistically significant (P<0.05). Conclusion IVIG combined with antiviral treatment can improve the laboratory indicators of children with Hand-foot-mouth disease disease and encephalitis,shorten the recovering time and course of various clinical symptoms,and the clinical effect is remarkable.

The Effect of Leukemia Cells on K+ and Glucose Detection in Peripheral Blood

Objective To explore if the leukemia cells count affect the K+ (potassium) and Glu (glucose) detection in PB (peripheral blood). Methods According to the clinical diagnose and the leukemia cells count,group the leukemia PB samples. K+ and Glucose concentration were detected. Leukemia cells were isolated from peripheral blood,labeled by PI and Annexin-V and detected by flow cytometry. Results Individual case indicated that pseudo-hyperkalemia may be present when WBC count is high. In PB samples no matter collected from acute or chronic leukemia patients,compared with control group (WBC<20×109/L),the K+ concentration increased significantly in WBC (20~50,50~100 and >100)×109/L groups,the Glucose concentration decreased significantly in WBC (50~100 and >100) ×109/L groups. Necrosis rate of leukemia cells increased significantly as in vitro time increased. K+ and glucose concentration change was not significant in 6 hours after separate serum from blood cells. Conclusion leukemia cells count affects K+ and Glucose concentration in PB,cell necrosis is part of the important reasons. Separation serum is one of effective way to avoid pseudo-hyperkalemia and pseudo-hypoglycemia in PB samples from leukemia patients.

Objective To evaluate the long term effect of Hepatitis B vaccination by analyzing the serum biomarkers among 0-17 year old children and young adults,so as to determine the necessity of booster vaccination. Method Quantitative analyses of serum HBsAg and HBsAb were performed in 42 905 children and young adults in the hospital.Results Thirty seven(0.087%) children and young adults showed positive HBsAg and 71.33% of them demonstrated positive HBsAb,with 93.41% in 1~2 year old babies but gradual decrease with age. The percentage of the subjects with the concentration of HBsAb over 100 mIU/mL increased from 34.21% in one-month old babies to 88.05% in 7-month ones. Nearly 80% of the subjects from the ages of 4~6 years to 18 years presented the stable level of HBsAg. Conclusion sThe prevalence of hepatitis B virus infection in children and young adults in Anhui is low.Serum HBsAg retest is helpful for evaluating the necessity of HB booster vaccination in preschool children and young adults.

Impact of Multimodal Analgesia on the Postoperative Cognitive Function and Concentrations of CPR,Cor and IL-10 in Orthopedics

Objective To reveal the influence of multimodal analgesia on postoperative cognitive dysfunction and C reactive protein (CPR),plasma cortisol (Cor),and IL-10 in orthopedics. Methods One hundred patients were divided into two groups,with 50 in each. The control group was treated with routine epidural analgesia,including 0.2 mg fentanyl,1.0 g tramadol,and 5.0 mg droperidol,mixed with physiological saline in total volume of 100 mL,while the experiment group were given multimodal analgesia,including 400 mg oral Celebrex,with 300 mg ropivacaine,40 mg methylprednisolone,5.0 mg Maca,and 0.5mL epinephrine,adding normal saline to 100 mL. The operation time,blood loss,intraoperative urine volume,POCD incidence,serum CPR,Cor,IL-10,VAS and MMSE score,adverse reactions rate in the two groups were examined and observed. Results No significant difference of operation time,intraoperative blood loss and intraoperative urine volume was noted between the two groups(P>0.05). The incidence of POCD in the experiment group was significantly lower than that in the control(P<0.05). The levels of CPR and Cor at 1h and 48h after operation were decreased compared with those of the control(P<0.05). The IL-10 concentration remained unchanged in the two groups(P>0.05).The VAS score was decreased(P<0.05) but MMSE score,increased(P<0.05)with less adverse reactions compared to the control(P<0.05). Conclusion Multimodal analgesia has a remarkable therapeutic role in postoperative analgesia of the orthopedics patients by reducing the incidence of cognitive dysfunction,the expression of inflammatory factors,and complications.

Objective To investigate the effect of glutamine on humoral and cellular immune function in children with severe HFMD. Methods 96 children with severe HFMD were enrolled in this study. They were randomly divided into three groups: group C (without glutamine enteral nutrition group,32 cases),group L (low dose 0.2 g / (kg•d) glutamine enteral nutrition group),and group H (high dose of 0.4 g / (kg • d) glutamine enteral nutrition group,32 cases),fasting morning blood on the first day,the eighth day and the fifteenth day of enteral nutrition support,The serum levels of IgA,IgG,IgM,CD4,CD4/CD8,and the nosocomial infection rate and hospital stay time in the three groups were recorded and compared. Results After the treatment,serum levels of IgA,IgG,IgM,CD4 and CD4/CD8 in three groups were significantly higher than those before treatment (all P<0.05),and The levels of serum IgA,IgG,IgM,CD4 and CD4/CD8 in group L and group H were significantly higher than those in group C (all P<0.05),The levels of serum IgA,IgG,IgM,CD4 and CD4/CD8 in group H were significantly higher than those in group C and group L (all P<0.05),all the differences were statistically significant. The infection rate and hospital stay in H group and L group were lower than those in C group (all P<0.05). The infection rate and hospital stay in group L were lower than that in group C (all P<0.05).Conclusion glutamine can improve the immune function of children,which can enhance the humoral and cellular immune function of children,reduce the damage of virus to children,and is worthy of popularization and application.

Irregular Antibodies to Blood Types and Their Influencing Factors in 3 078 inpatients

Objective To analyze the distribution of irregular antibodies against blood types and the influencing factors of gender,age,blood transfusion or pregnancy history in hospitalized patients.Methods The blood samples of 3078 inpatients were collected,the manual polybrene tests were used for irregular antibody screening,the panel cells were used for irregular antibody identification to analyze the results and influencing factors. Results Thirty-two cases of 3078 inpatients were found to carry irregular antibodiesaccouted for with the positive rate of 10.396‰. Among them,the alloantibodies accounted for 84.38% whereas the autoantibodies,15.625%,after identification.The antibodies to Rh,MNSs and Lewis blood groups accounted for 43.75%,15.63% and 12.50%,respectively. The frequencies of IgG,IgM and IgM-IgG classes were 62.50%,31.25% and 6.25%,respectively. Univariate analyses showed that the positive rates of irregular antibodies in female,patients aged over 60 years,blood transfusion or pregnancy history were significantly increased when compared with the other patients investigated (P<0.05). Multivariate logistic regression analysis showed that age (over 60),blood transfusion and pregnancy were the main influencing factors of irregular antibody production (P<0.05). Conclusion The level of serum irregular antibodies in hospitalized patients of Huainan district was positively related to age,history of blood transfusion and pregnancy.
