Raman Spectroscopy Analysis of Hemoglobin Conformation and Its Correlation with P50 Values

Objective To explore the difference of the red blood cells (RBCs) in Raman spectrum and P50 values in people living in the plain and the plateau for various durations and to analyse the effect of hypoxia on the structure of human hemoglobin. Method Thirty healthy males were selected,with 15 in each. The peripheral anticoagulant blood samples were collected and the hemoglobin was used for detection by Raman spectrum and suspension of the RBCs was subjected to the P50 value determination. Results Compared with those residing in the plain,distinct peaks of Raman spectrum were noted on the locations of 1 341 cm-1,1 375cm-1,1 585 cm-1,and 1 638 cm-1,respectively,after 3 and 7 days in the men of rush entrance into the plateau. After 30 days of acclimatization training on the plateau,the characteristic peaks of the spectrum disappeared. The P50 value increased with the rapid entry to the plateau from 3 to 7 days whereas decreased following acclimatization for 30 days. Conclusion The hemoglobin structure modifications exist in those who have a rush advance to the highland,with the P50 value increasing.

Effects of Different Blood Storage Methods on Oxygen Carrying Function of Red Blood Cells

Objective To evaluate the effects of the populations of the Tibetan and Han Plain on the function of red blood cells carrying oxygen in different storage conditions. Method sAnticoagulation was collected from the populations of 7 the Tibetan and 6 Han Plain and red blood cells were saved with different preservation solutions (4℃ EDTA anticoagulant,4℃ red blood cell preservation solution,compound glycerol solution -80℃). After three days red blood cell P50 was detected under the above conditions. The pH of the 4℃ EDTA anticoagulant was also detected for analyzing the reasons that may affect P50. Results The P50 datas of all specimens of Tibetan and Han nationality were lower than those of normal people. The P50 data in the preservation solution at 4℃ was higher than those in the 4℃ EDTA anticoagulant and whole blood of the frozen solution which was stored in glycerol solution -80℃. The P50 data of the red cell of the Tibetan stored in the preservation solution at 4℃ was higher than that of Han Plain. At the same time,there is no significantly difference in the P50 data of whole blood which was stored at 4℃ or-80℃ ConclusionDifferent preservation solutions could have significant effect on P50 data of red blood cells. There is no difference between the three different preservation solutions on the P50 of Tibetan an Han population. The P50 of red blood cells stored in the CAPD-1 preservation sloution of the two groups were relatively high,and the preservation solution had little effect on the red blood cell carrying oxygen. Therefore the above conclusions will provide guiding significance for blood preservation and transfusion of patients,especially for high altitude blood transfusion.

Research on The Oxygen Affinity P50 of Red Blood Cell Containing L-Arginine

Objective To evaluate the effects of different L-arginine concentration on the oxygen affinity P50 of red blood cells and frozen red blood cells in different storage periods. Method sL-arginine solution was added into the CPDA or MAP preservation solution after thawing of frozen red blood cells,before preservation of frozen red blood cells and red blood cells,to make the final concentration of L-arginine 5 mmol/L,10 mmol/L and 50 mmol/L. The changes of erythrocyte oxygen affinity P50 were detected at 0d,7 d,14 d,21 d,28 d and 35 d during the storage periods,respectively. Results With the extension of the preservation periods,the overall decrease of erythrocyte oxygen affinity was observed in L-arginine solution was added into the CPDA or MAP preservation solution after thawing of frozen red blood cells,before preservation of frozen red blood cells and red blood cells. Especially during the first two weeks of the preservation periods,the decrease was significant.Decreased by 15.6% over 14 d,then it fell more slowly,and day 35 was down 25.4%.At the end of the inventory period,the value is only 74.6% of the date of blood collection; The overall decrease of erythrocyte oxygen affinity was observed in frozen red blood cells.The effect of L-arginine on oxygen affinity P50 of frozen red blood cells was greater than that of suspended red blood cell,and the effect of L-arginine addition was most significant after thawing of frozen red blood cells; L-arginine with a final concentration of 10mmol/L had the most significant effect on the improvement of erythrocyte oxygen affinity P50 in the three groups,and the difference was statistically significant. Conclusion L-arginine at a suitable concentration can improve the oxygen affinity P50 of red blood cells and frozen red blood cells.

Effect of Platelet Storage Lesion on The Clinical Efficacy of Transfusion

Objective To investigate the relationship between platelet storage lesion and treatment efficacy of platelet transfusion. Method sThe platelet aggregation function and P-selectin were measured in 64 patients with hematologic disorders. The platelet recovery rate (PPR),platelet increase index (CCI) and hemostasis were measured for evaluation of transfusion efficacy and the correlation between platelet storage lesion and platelet recovery was analyzed. Results The effective CCI value of platelet transfusion was (16.0±5.5) in 40 patients,the CCI value of 24 cases was (3.4±1.0),the median of maximal platelet aggregation rate(Mean%±SD%),the minimum value (min%) - maximum value (max%) were 48.1±4.7(41.1~56.2),38.1±3.5(31.2~43.1),17.4±2.4(14.5~20.9),and 9.6±3.3(5.8~19.9) from 2d to 5d of validation. The contents of plasma soluble P-selectin (Mean±SD) were (11.4±5.8),(28.9±7.5),(55.5±8.4),and (89.2±5.6)ng/mL in the period of validation. A significant positive correlation was seen between platelet aggregation function and platelet transfusion efficacy (r=0.892,P<0.01),while a marked negative correlation between plasma soluble P-selectin concentration and platelet transfusion efficacy (r=–0.836,P<0.01). Conclusion Platelet injury during the period of platelet preservation may affect the incidence of platelet transfusion inefficiency. The transfusion efficacy can be improved by incorporating the functional criteria of apheresis platelets into clinical study.

A Feasibility Study on Preparation of Cryoprecipitation with Dewhite Plasma

Objective To investigate the feasibility of the cryoprecipitation using the dewhite plasma within 24h . Method A total of 180 bags of blood (400mL/bag) were collected and stored under 22℃ from May 2016 to February 2017. The blood was divided into six groups (A1,A2,B1,B2,C1,C2),with 30 bags in each. The concentrated platelets were prepared by albuginea method within 8 h after collection in group A1 and within 20 h-24 h in groups A2,B1,B2,C1 and C2,respectively. The upper-albuginea plasma was separated and quickly frozen as the source for cryoprecipitation. The contents of factor VIII (FVIII) and Fibrinogen (FIB) in cryoprecipitation were detected by coagulation analyzer,and the influence of different preparation time (groups A1 and A2),quick-freezing methods (groups B1 and B2) and cryoprecipitation preparation methods (groups C1 and C2) on the contents of FVIII and FIB in cryoprecipitation and the qualified rates were compared. ResultThe influence of two storage times on the quality of FVIII and FIB in cryoprecipitation was not statistically significant (P>0.05). The freezing by instant freezers and preparation methods of cryoprecipitation gave rise to a remarkable impact on the quality of FVIII(P<0.05). The FIB in groups A1,A2,B1,B2,C1 and C2 was qualified but a statistical difference (P<0.05) was noted FIB between groups C1 and C2. Conclusion The dewhite plasma within 24 h at 22℃ after concentration of platelets might be used as the source for cryoprecipitation,the temperature of the plasma and preparation method of cryoprecipitation play a crucial role in guaranteeing the quality of FⅧ and FIBs.

Compilation of Voluntary Blood Donation Intention Questionnaire and Examination of Its Reliability and Validity

Objective To construct a reliable and effective questionnaire for assessment of the blood donation intention in community residents and to explore its influence factors based on the Theory of Planned Behavior for formulating donation recruitment strategy. Method sThe good-poor analysis,stepwise regression analysis,correlation analysis,Cronbach's ɑanalysis,and factor analysis were used to evaluate the items. Cronbach's alpha coefficient and confirmatory factor analysis was used to evaluate the reliability and validity of the questionnaire. Results The voluntary blood donation intention questionnaire was composed of 9 dimensions and 23 items. Alpha coefficient of total questionnaire was 0.889 and 0.644 to 0.917 for each dimension. Confirmatory factor analysis showed that the questionnaire fitted well. Conclusion Each item of this questionnaire has satisfied sensitivity,independence and representativeness. The high reliability and validity of this questionnaire can be used to assess the donation intention of community residents.

A Comparison of Postoperative Transfusion Strategies in Elderly Patients with Hip Fracture

Objective Hip fracture (HF) in frail elderly patients is associated with poor physical recovery and death. Postoperative blood loss is frequently seen and the volume of red blood cell (RBC) transfusions is unknown in the patients. Method We investigated the association of RBC transfusion strategies with the degree of physical recovery or with reduced mortality after HF surgery. We collected 115 consecutive postsurgical HF patients (age≥65 years) with Hb levels<113 g/L in the hospital. Allocation was stratified by residence. The patients were randomLy assigned to either restrictive (Hb<97 g/L) or liberal (Hb<113 g/L) RBC transfusions given within the first 30 days postoperatively. Follow-up was performed for 90 days. Results Statistically significant differences were found in repeated measures of daily living activities or in 90-day mortality rate between the restrictive group (30% died) and the liberal group (16% died). Thirty-day mortality was higher with the restrictive strategy (hazard ratio (HR) =1.7 95%CI 0.6-3.4; P=0.02) compared with that with liberal transfusion. The 90-day mortality rate was higher for patients in the restrictive transfusion group (36%) than for those in the liberal group (20%) (HR =1.5 95%CI 0.8-2.1;P=0.03). Conclusion According to our Hb thresholds,recovery from physical disabilities in the patients with frail elderly hip fracture varied following restrictive RBC transfusion strategy and liberal strategy. Implementation of a liberal RBC transfusion strategy in nursing home residents has the potential in increasing survival.

Analysis and Discussion on the Return of Unpaid Blood Donors in Wuhu Area

Objective To explore the method and feasibility of returning reactive blood donors and seek the balance between blood loss and blood safety. Method sScreening of previous blood donors who meet the conditions for returning to the site to re-collect specimens (Screening of always eligible for this site back to collect blood donors specimens) (time interval ≥6 months ). Routine ELISA and NAT detection first,if there is no reactivity,the blood will be sent to the provincial blood bank for confirmation experiments and supplementary experiments.(makes the province blood center and added Confirmatory tests). Results From January 2014 to April 2008,a total of 166 recalled blood donors were selected,of which 63 specimens were unqualified in routine testing,and unqualified items were the same as previous screening items.(Dating from January 2014 to April 2018,a total of 166 cases of recalled blood donors were recalled,among which 63 cases of samples failed the routine test of this site,and the unqualified items were the same as the previous shielding items.) 103 cases of non-reactive specimens which detected by ELISA and NAT were sent to the provincial blood center. 83 of them were confirmed to be negative and could return to the team. (All 103 cases of ELISA test and NAT test were sent to the provincial blood center without reactive samples,of which 83 cases were confirmed negative and could be returned to the team. ) 31of the 83 people donated blood again and 13 of them donated blood several times. (In 83 case,31 cases were again donate blood and 13 cases were donated multiple times.) ConclusionAt present,the ELISA test and the NAT test combined with the confirmatory test could ensure blood safety to a large extent.(the combination of ELISA test and NAT test with confirmatory test ensures the safety of blood to a large extent. )This makes it possible for some of the fake reaction donors,especially the gray area and the weakly positive fake blood donors,to return to the blood donor team again,(It also enables some blood donors with false-reactive,especially those in grey zone and weak positive ones,to return to the blood donor team again,)which is of great significance for promoting the development of voluntary blood donation.

Objective To investigate the genetic polymorphism of HPA-15 alleles in Kazak blood donors from Urumqi area and to provide valuable data for clinical platelet transfusion. Method sHPA-15 system genotype was determined by PCR-SSP method in 100 Kazak blood donors from Urumqi. The frequencies of gene and genotype were calculated and compared with that in other areas. Results The frequencies of HPA-15aa,HPA-15ab and HPA-15bb in Kazak blood donors were 0.21,0.53 and 0.26,respectively. The gene frequency of HPA-15a and HPA-15b were 0.475 and 0.525,respectively. The frequencies of antigen incompatibility of HPA-15a and HPA-15b were 0.1997 and 0.1747,respectively. Conclusion The frequency of antigen incompatibility of HPA-15 is higher,which indicate HPA-15 may increase the probability of human platelet alloimmunization and may play an important role in clinical platelet transfusion.

Application of Nucleic Acid Test in Blood Screening of Donators

Objective To investigate the necessity of nucleic acid test (NAT) and its significance in blood screening of donators in Yancheng district. Method sA total of 55 186 blood samples with at least one negative ELISA were selected in 2015. The NATs were performed in all samples based on Roche Cobas s201 nucleic acid amplification testing system (Roche system),the HBV DNA,HCV RNA and HIV RNA were individually detected with DAAN system in the pooled blood of 6 or 8 samples. The positive samples in the screening were subjected to confirmatory tests in the Province Blood Center. Results Totally,7 965 pools were tested by NATs in the samples that showed at least one negative ELISA. Among them,47 (1.20%) positive pools of 3 930 pools were found by Roche system. In the 4 035 pools tested by DAAN system,39 (0.97%) positive pools were noted. 42(0.076% ) samples were found to be positive in individual NATs. The positive rates of Roche system and DAAN system were 0.12% and 0.047%,respectively (P<0.05). In 36 samples,25 (69.44%) were confirmed to be HBV DNA positive,while no HCV RNA and HIV RNA were found. Conclusion Nucleic acid test is recommended following serological detections,which would cover the shortage of serological tests and ensure the blood safety.

A Comparative Study of Two Profile Detections for Unqualified Blood Donations

Objective To analyze the effects of two different detection modes on the effectiveness of blood screening from 2010 to 2015. Method sThe retrospective and chi-square test were used to analyze the number of donators and volumes of blood collected and discarded. Results Mode 2 composed of gold labeled fast detection for HBsAg,dry chemical method for ALT,ABO typing,physical examination of the donors,followed by re-detections of nucleic acids and antibodies to ALT,HBsAg,HCV,syphilis,HIV,and ABO blood group and RHD type etc. A total of 1 546 938 blood donors were collected . Among them,5.19% and 3.82% of blood were discarded when examined by the two modes of profile detections due to positive nucleic acids or antibodies against ALT,HBsAg,HCV,and syphilis. A significant difference of positive rates between the two modes was noted. Conclusion Mode 2 is appropriate for detection of infectious agents and effective for reduction of blood wastage.

Determination of the Grey Zone of ELISA in Anti-HIV Detections

Objective To explore the rationality of the grey zone determination in anti-HIV detections by ELISA in screening of blood donation. Method sA retrospective analysis was performed with anti-HIV positive and grey zone (0.5≤s/co<1) specimens that were subsequently subjected to Tongling Center for Disease Control for confirmation from January 2014 to December 2017. The ROC curve was used to compare the sensitivity and specificity of the reagents under different s/co conditions. Twenty negative specimens of anti-HIV(s/co<0.5 ) were randomly selected for WB tests. Results A total of 39991 specimens were tested by the reagents from Wantai company,and 26 positive specimens and 22 grey zone specimens were found. Fifteen positive and 7 grey zone specimens were detected out by the reagents provided by Lizhu company. Among them,however,only 5 specimens were confirmed to be positive by Tongling CDC. Conclusion The over-sensitive values of s /co recommended by the reagent manufacturers for blood screening is able to meet our requirements and the setting of grey zone may cause unnecessary waste of donated blood.

Expression of Endometrial LIF and LIFR in Women with Primary Infertility

Objective To explore the expression of Leukemia inhibitory factor(LIF )and Leukemia inhibitory factor receptor (LIFR) signal in the primary infertile women endometrium with different causes during the implantation window and the relationship between LIF、LIFR and unexplained infertility. Method sIn the prospective clinical controlled study,200 patients with infertile from Jan.2015 to Jan.2017 in our hospital were divided randomly into tubal factor(group A),poor ovarian reserve(group B),endometriosis (group C) and unexplained infertility (group D) according to different pathogenesis.Then,normal women were selected during the same period as the control group,the endometrial expression of LIF and LIFR were detected by fluorescence quantitative PCR,while the differences were compared between groups. Results The expression level of LIF in the control group was (1.12±0.46),while in A,B,C and D group were (1.03±0.33),(0.90±0.30),(0.99±0.38) and (0.50±0.31),respectively.There was no significant difference in the expression of LIF between the three groups of A,B,C and the control group,but the expression level of LIF in D group was significantly lower than that of the control group(P<0.01). The endometrial expression of LIF was significantly decreased in D groups than that of the other infertile groups.The endometrial LIFR in four infertile groups were all significantly decreased (P<0.01),while there was no statistical difference between the subgroups (P>0.05). Conclusion The endometrial LIFR expression in general infertile women might be impaired,which deduced that LIFR might be one of the key factors for the pregnant regulation in female endometrial planting window. Deletion of LIF may be associated with unexplained infertility.

The Significance of Low Hemoglobin Density Combined with RBC Volume Distribution Width for Diagnosis of Small Cell Hypochromic Anemia of Children

Objective To investigate the value of low hemoglobin density (LHD) combined with red blood cell volume distribution width (RDW) for the diagnosis and treatment of microcytic hypochromic anemia (MHA) in child patients. Method One hundred and two patients with MHA were collected and 120 healthy children were taken as the control. Blood routine tests of the all samples were performed with Sysmex XN9000 Cell Analyzer before and 3 months after treatments. LHD was obtained based on average hemoglobin concentration (MCHC) and both LHD and RDW were statistically analyzed. Results The values of LHD (32.57%±18.62%)and RDW (17.06%±3.54%) in the patients before treatments were found to be obviously higher than those in the control(LHD:2.97%±0.93%; and RDW:12.01%±2.56%) (t=4.287~21.052,P<0.05~0.01). The high LHD value was mostly noted in males but not females(t=4.029,P<0.05). LHD and RDW values after treatments,however,were 11.32%±9.74% and 13.85%±2.73%,respectively,which was notably lower than those before treatments(t=3.571~10.187,P<0.05). No statistical difference was seen in gender (t=0.592~1.095,P>0.05). Additionally,the concentration of both LHD and RDW increased with the severity of anemia . Conclusion The values of LHD and RDW in the patients with MHA increased with the severity of the disease but significantly decreased following treatments,suggesting their significance in monitoring of MHA.

Objective To analyze the correlation between plasma homocysteine (HCY) level and carotid artery related risk indicators (such as carotid atherosclerotic plaque and CIMT) in acute ischemic stroke patients with H-type hypertension. Methods 115 cases of acute ischemic stroke complicated with H-type hypertension were selected as group A,68 cases of acute ischemic stroke complicated with simple hypertension as group B and 110 cases of acute ischemic stroke as group C. Systolic blood pressure,diastolic blood pressure,homocysteine (HCY),blood lipid levels were measured and carotid plaque and CIMT were detected in the three groups; the correlation between blood pressure,HCY level,blood lipid,carotid plaque and CIMT were analyzed. Results There was no significant difference in TC,LDL-C and HDL-C between the three groups (P>0.05); SBP,DBP and TG in group A were higher than those in group B and group C with statistical difference (P<0.05); SBP and DBP in group B were higher than those in group C with statistical difference (P<0.05); there was no significant difference in the distribution of TG in group B and group C (P>0.05). Hcy and CIMT in group A were higher than those in group B and group C (P<0.05),and HCY and CIMT had no statistical difference between group B and group C (P>0.05). Carotid plaque was detected between the three groups as χ2=13.346 (2=13.346) and bilateral P=0.001,and the difference was statistically significant (P<0.05) according to the level of alpha=0.05. Pearson linear correlation analysis showed that HCY level was positively correlated with CIMT and carotid plaque in the cases of acute ischemic stroke complicated with hypertension (r=0.545,P<0.01). Conclusion HCY in the cases of acute ischemic stroke complicated with hypertension are positively correlated with such common carotid artery risk indicators as CIMT and carotid plaque formation. H-type hypertension is an independent risk factor for increased CIMT and plaque formation. Detection of HCY,CIMT and atherosclerotic plaque plays an important role in evaluation and treatment of acute ischemic stroke complicated with H-type hypertension.

Objective To investigate the clinical value of serum CXCL14,TNF-α,KL-6 and SP-D in the diagnosis of idiopathic pulmonary interstitial fibrosis. Method sForty patients with idiopathic pulmonary fibrosis from January 2015 to December 2016 were selected as the study group,and 42 patients with bacterial pneumonia were enrolled in the same period. as a control group. CXCL14,TNF-α,KL-6 and SP-D were measured by enzyme-linked immunosorbent assay (ELISA). The serum levels of CXCL14,TNF-α,KL-6 and SP-D were compared. Results The four indexes of the study group were higher than the threshold (predetermined value),the four indicators of the control group were lower than the threshold,and the difference between the two groups was statistically significant(P<0.05).The positive rate of the four parameters was significantly higher than that of the control group (P<0.05)in the separate test. The positive rate of the two groups was 90.0% in the combined test,which was significantly higher than that in the control group(P<0.05). Conclusion The expression of CXCL14,TNF-α,KL-6 and SP-D in the patients with idiopathic pulmonary fibrosis is high,and the combined detection of four indicators can improve the diagnosis of idiopathic pulmonary interstitial fibers.

A Combination of β-catenin/Cadherin Examination with Serum Uric Acid for the Prognosis of Diabetic Nephropathy

Objective To explore the value of combined determination of β-catenin/cadherin together with serum uric acid for the prognosis of diabetic nephropathy (DN). Method sOne hundred and forty-six DN patients were selected in the past one year and divided into the groups of CKD stage 1~3 (88 cases)and CKD stage 4~5 (58 cases)according to glomerular filtration rate (GFR)and serum creatinine level (Scr). Logistic analysis was used to explore the independent risk factors affecting the renal function of DN and the clinical significance of β-catenin/cadherin detection combined with serum uric acid (UA)for evaluation of renal function. Results No significant difference of the assessment of DN prognosis was noted in gender,age,BMI,smoking history,FPG,HbA1C,TC,TG,HDL-C and LDL-C levels (P>0.05). The levels of serum UA,urinary protein,HMOA-IR,urinaryβ-catenin/cadherin varied significantly in DN patients with different prognosis (P<0.05). Logistic regression analysis showed that the concentrations of UA,urinary protein,HMOA-IR,and β-catenin/cadherin were independent risk factors which affect renal function. The patients with CKD stage 1~3 presented a normal result of the combination detection of UA andβ-catenin/cadherin(P<0.05)whereas the patient with CKD stage 4~5 demonstrated a high value of the combination tests(P<0.05). Conclusion sTheβ-catenin/cadherin and serum UA are independent risk factors of renal disfunction and a combined detection of the three markers is clinically valuable in the prognosis of DN patients.

Objective To investigate the diagnostic significance of combined detection of NAP,PCT and CRP in orthopedic postoperative infection. Method sThree hundred and four inpatients who had received Class Ⅱ treatments were divided into infection group (262 cases) and non-infection group (42 cases) from June 2016 to January 2018. 5 mL of venous blood was collected on the first day postoperation. PCT was detected by double-antibody sandwich immunochemiluminescence assay,CRP was tested by immunofluorescence dry-quantitative assay,and NAP assay was carried out with NAP substrate-AMP buffer method. Antibiotic sensitive tests were performed with disc diffusion assay (K-B method). The ROC curves were analyzed according to the expression levels of NAP,PCT and CRP. Results Among 304 orthopedic patients,42 (13.8%) had postoperative infections,from whom 89 strains of bacteria were isolated that included 39 (43.8%) G-positive and 37 (41.6%) G-negative bacteria,4 (4.5%) viruses,6 (6.7%) mycoplasma,and 3 (3.4%) unidentified microbes. The infection group was significantly higher than the non-infection group in terms of NAP scores,NAP positive rate,PCT and CRP (P<0.05). The combined detections of NAP,PCT,and CRP for postoperative orthopedic infection was AUC:0.928,which was significantly higher than that of NAP,PCT,and CRP (AUC:0.667,0.779,0.877,P<0.01),suggesting that joint detections of NAP,PCT,and CRP help with the diagnosis in orthopedics postoperative infection. Conclusion Combined detections of NAP,PCT and CRP are clinically valuable in the diagnosis of orthopedic postoperative infection.

Objective To investigate the changes of peripheral blood T-lymphocyte subsets, serum procalcitonin (PCT) and high-sensitivity-C-reactive protein (hs-CRP) levels in elderly patients with stroke-associated pneumonia (SAP). Method sFifty-nine patients with senile stroke admitted to hospital within 24 hours from February 2017 to February 2018 in our hospital were selected as study subjects. 35 patients with SAP within 1 week were included in group A, and 24 patients without SAP were included in group B in this study; 45 cases of physical examination in our hospital at the same period were used as control group. Peripheral blood T lymphocyte subsets, serum PCT and hs-CRP levels were measured and compared. Results The levels of CD3+, CD4+, and CD4+/CD8+ in group A and B were lower than those in control group. The level of CD8+ was higher than that in control group (P<0.05). The ratio of CD4+/CD8+ in group A was lower than that in group B (P<0.05); Serum PCT and hs-CRP levels in group A and B were significantly higher than those in control group (P<0.05); Serum PCT and hs-CRP levels were higher in group A than in group B, the difference was statistically significant (P<0.05). Conclusion sThe elderly patients with SAP have lower levels of cellular immune function at the early onset, and serum PCT and hs-CRP levels are higher. Serum PCT and hs-CRP show significant clinical value for the detection of early stroke-associated pneumonia

A Predictive Value of Serum Hypersensitive C-reactive Protein in Restenosis Following Carotid Artery Stenting

Objective To investigate the relationship between serum hypersensitive C-reactive protein (hs-CRP) and carotid artery stenosis and restenosis after carotid artery stenting (CAS),so as to provide references for its clinical application. Method sOne hundred and sixty-eight patients undergoing CAS were selected in the hospital. The level of serum hs-CRP of pre-operation and 1d,7d,3 mon and 6 mon post-operation was measured. The relationship was analyzed between serum hs-CRP level and the degree of stenosis and restenosis following CAS. Results The incidence of carotid stenosis increased with the level of serum hs-CRP (P<0.05) before operation. The incidence of restenosis was 19.05% (32/168) after 6 months of operation. No difference of the serum hs-CRP level was noted at pre-operation and 1d post-operation between the patients of restenosis and non-restenosis (P>0.05). The serum hs-CRP level in the patients of restenosis was significantly higher than that in those of non-restenosis on 7 d,3 mon and 6 mon post-operation,while the patients of non-restenosis had a higher level of △hs-CRP than those of restenosis (P<0.05). The serum hs-CRP level in all the patients increased significantly on day 1 after surgery,and it remained a high level after 7d of surgery in the patients of restenosis compared with those of pre-operation. However,the hs-CRP level was significantly decreased in the patients of no-restenosis (P<0.05). Multivariate regression analysis showed that △hs-CRP was an independent risk factor for restenosis after CAS. Conclusion The remarkable increase of serum hs-CRP was positively associated with the carotid stenosis following CAS,suggesting a predict marker for carotid artery restenosis.

Objective To analyze the risk factors of thrombogenesis in the elderly population through functional tests of platelet aggregation. Method Tests of the maximum aggregation rate (MAR)and average aggregation rate (AAR) were carried out in 134 old persons. The impact of the age,gender,GLU and LDL on MAR and AAR and the value of their difference was analyzed. Results The ratio over 80 percent of MAR value was 39.6%,which is obviously higher than that (14.3%) in the control,and the ratio in females was higher than that in males (P<0.05). The median difference (3.90) of females was lower than that of males (4.85). No impact of blood glucose (P>0.05) but marked influence of LDL(P<0. 01)was noted on the abnormal value of MAR. Additionally,the value of LDL was closely correlated with the difference of MAR-AAR(R=-0.824,P<0.05). Conclusion The risk factors of age,gender,as well as LDL level have notable impact on platelet aggregation.

Objective To evaluate the performance of TOSOH HLC-723 G8 analyzer for clinical detection of HbA1C. Method sPrecision verification was based on EP15-A2 and WS/T461-2015 methods. Correctness verification was compared with the results of Bio-Rad D10 reference detection system. The WS/T420-2013 verification method was used for linear verification,and the carrying rate was three times with high and low values of continuous sampling. Results The low value of short term precision was 0.86% and the high value,0.98%. The long-term precision low value was 0.98% and the high value,0.69%,which is highly correlated to the results tested by the reference instrument (r=0.999,r2=0.998,P=0.000). Additionally,a good linearity in the range of 5.2% and 15.2% (y=1.0115x-0.0505,r=0.9997) and low rate of carryover(k=1.00%) were noted. Conclusion TOSOH HLC-723 G8 has a high precision,good linearity and low pollution rate and can be used for clinical detection of HbA1C.

Objective To analyze the clinical application value of serum procalcitonin (PCT) combined with high sensitivity C-reactive protein (hs-CRP) in neonatal intestinal infectious diseases and its effect on prognosis. Method s60 cases of neonates with intestinal infectious diseases admitted in our hospital from April 2016 to July 2017 were collected as the observation group. The 60 cases of uninfected newborns were selected as Non-infected group at same period. Detection and comparison of two groups of serum PCT and hs-CRP and other indicators of the level of the situation. Results The levels of PCT and hs-CRP in the infected group were significantly higher than those in the non-infected group (P<0.05). The above indexes were effectively down-regulated (P<0.05) after the clinical treatment,the diagnostic sensitivity and accuracy of intestinal infectious diseases are better than serum CRP. Conclusion PCT test can significantly improve the diagnostic accuracy of neonatal intestinal infectious diseases,guide the follow-up treatment,and improve the quality of prognosis,which have a positive effect.

Objective To explore the value of CRP/PA ratio in the early clinical diagnosis and treatment of chronic obstructive pulmonary disease. Method sCRP,PA,RDW,WBC,N% and other indicators were detected before and after treatment,as well as serum C-reactive protein (CRP) and prealbumin (PA ratio) in COPD of 50 patients with acute exacerbation of patients with stable COPD group,50 cases in the control group in the Ma'anshan Market Center Hospital from March 2015 to March 2016. Results CRP was significantly higher in the study group before treatment than that in the control group after treatment,CRP is still higher than the control group after treatment (P<0.05); the study group before treatment was significantly lower PA than that of after treatment and control group after treatment control group (P<0.05); C-reactive protein (CRP) and prealbumin (PA) ratio of 0.853 the study group before treatment; the positive rate of CRP (84%),PA (76%),WBC (26%),RDW (62%),N% (72%) was significantly higher than that after treatment and control group (P<0.01). Conclusion CRP/PA ratio has an important guiding significance for the early diagnosis of AECOPD,which provides strong evidence for effective medical clinical application of antibiotics.

Objective To validate the applicability of the reference intervals of reticulocyte parameters provided by Sysmex XN3000 to the local adult population. Method sOne hundred and sixty healthy persons were collected for the individual examination of seven item parameters of reticulocytes according to the Reference Intervals of Laboratory Test Items as Professional Standards (WS/T402-2012),which include absolute value of reticulocytes(Ret#),percentage of reticulocytes(Ret %),percentage of hyper-fluorescent reticulocytes(HFR%),mid-fluorescent reticulocytes (MFR%),low-fluorescent reticulocytes(LFR%),immature fluorescent reticulocytes(IRF),and hemoglobin of reticulocytes (Ret-He). Results All the persons who received the examinations passed the verification of the 7 items. Conclusion sThe parameters of Ret#,Ret%,LFR%,IRF,and Ret-He were obtained at the same time although the HFR% and MFR% limit to the standard within 10 percent at maximum,suggesting that the standard parameters would be needed for examination of reticulocytes in local adult population.

Objective To observe the clinical effect of closed negative pressure drainage (VSD) combined with silver ion irrigation in the treatment of infection wounds in Department of orthopedics. Method s106 cases of wound infection in Department of orthopedics in our hospital from September 2015 to March 2017 were randomLy divided into observation group and control group,53 cases in each group,the observation group was given VSD combined with silver ion washing treatment,the control group only received routine VSD treatment,observe the therapeutic effect and complications,record control time and hospitalization time two the frequency of dressing,infection,respectively before treatment,after 3 weeks of treatment in two groups were detected with white blood cell count,erythrocyte sedimentation rate and C-reactive protein C. Results The treatment effect of observation group was higher than the control group,the number of dressing changes,infection control time,hospitalization time,complication rate,leukocyte count,erythrocyte sedimentation rate and C-reactive protein C were lower than the control group,the difference was statistically significant (P<0.05). Conclusion The application of VSD combined with silver ion irrigation in the treatment of wound infection in Department of orthopedics can improve the treatment effect,shorten the cure time of patients,and promote the patients to leave hospital as soon as possible.
